Honest Taskers, Teem, and Virtual Latinos lead this year's shortlist of companies to hire a medical virtual assistant from Latin America, seven firms ranked on whether they state their talent is Latin American, what they charge, how the time-zone overlap with US hours works, and whether anyone will sign a BAA. That last question separates this region more than price does, and the answer is uncomfortable. Below we set out the choice between Latin America and the Philippines, which HIPAA gap to check, what the work costs, what a company should put in writing, and whether hours or Spanish is your reason.
Disclosure: Honest Taskers publishes this list and appears at position 1. We're judged on the same five published criteria as every other company here.

How we chose these companies
Latin America has become the default recommendation for practices that tried offshore support and disliked the time difference. The region's central advantage is structural, since Bogotá, Mexico City, and São Paulo all sit within a few hours of US business hours, so your assistant works days rather than nights. Add Spanish as a first language for most of the talent pool and you have a genuinely different proposition from Asian outsourcing rather than a cheaper version of it. We compared five things across every firm, which are stated talent location, published pricing, the healthcare roles each company names, time-zone and language capability, and HIPAA posture including whether a BAA is signed.
That last criterion produced the most useful finding in this vertical, and it isn't flattering to the region. Of the seven firms here, only two make a clear HIPAA statement. Virtual Latinos describes training as "HIPAA-conscious" and mentions NDAs while describing no signed BAA. Remote Leverage and WOW Remote Teams say nothing about HIPAA at all, despite both selling healthcare roles, and Remote Leverage itemizes medical billing, prior authorization, and EHR management specifically. None of that makes them bad firms, but it does mean the compliance conversation is yours to start, and you should assume nothing.
Location was the first filter, as across this series. Because these titles ask for talent "from Latin America" rather than from one country, a firm that states LATAM as its talent base qualifies even without naming countries, which is a fairer bar than a single-country article would set. Firms that publish no location at all were excluded. The U.S. Department of Health and Human Services sets out what a business associate arrangement requires, and it's worth reading before you sign with anyone here.

1. Honest Taskers
Best for: practices that want a Latin American assistant working their own hours, with a BAA and a published rate.
Pricing: $10.00–$12.65/hrLocation: LatAm, PH, India, PakistanModel: Managed staffingFirst hire: Two-week working trial
Honest Taskers recruits in Latin America and the Philippines, so you can ask for a Latin American assistant specifically and get the daytime overlap that makes this region worth paying slightly more for. The role covers the full administrative load, including patient calls, scheduling and confirmations, insurance verification, prior authorizations, records, refill coordination routed to your clinicians, and billing support, inside your own EHR. On compliance it's the clearest firm in this region by a distance, with staff trained on HIPAA and data privacy under a dedicated compliance officer, a Business Associate Agreement signed where they'll access PHI, and HIPAA compliance verified by Accountable. Rates run $10.00 to $12.65 an hour, the first hire includes a two-week working trial, and the company reports 99.6% average monthly retention. Its pool also includes licensed nurses and physicians.
Limitation: it recruits across four regions, so confirm your hire comes from Latin America if the time-zone overlap is what you're buying.
2. Teem
Best for: practices with Spanish-speaking patients that want a published rate and a guarantee a person answers.
Pricing: $11.50/hrLocation: Latin AmericaModel: Full-time placementTrial: Not publicly listed
Teem states that most of its team sits in Latin America and ties that directly to Spanish-language demand among US clients, which is a more honest framing than treating bilingualism as a bonus feature. It publishes $11.50 an hour, worked out from a $1,995 monthly fee at 174 working hours, and it's unusually direct about staffing humans, stating that every call is answered by a real person rather than AI and that human scribes catch context automated tools miss. It also absorbs the legal, compliance, and payroll administration for the assistant, which removes a real chunk of employer overhead from a small practice.
Limitation: the highest published rate in this region, and its HIPAA position is described as training rather than a signed BAA.
3. Virtual Latinos
Best for: practices that want a monthly full-time price and a provider built entirely around Latin American talent.
Pricing: $1.8K–$3.7K/mo full-timeLocation: Latin AmericaModel: Managed placementTrial: Not publicly listed
Virtual Latinos is Latin America only, which means the region isn't one option among several but the whole business, and it runs a healthcare and medical line covering patient scheduling and billing support. Pricing is published as a monthly full-time figure of $1.8K to $3.7K for 160 hours a month, tiered by experience, which works out competitively per hour while being easier to budget than an hourly rate. Bilingual capability and US time-zone alignment are both standard rather than upsells. Assistants sign NDAs and training is described as "HIPAA-conscious".
Limitation: no signed BAA is described publicly, and "HIPAA-conscious" is not the same as HIPAA-compliant, so get the agreement in writing before any PHI moves.
4. Remote Leverage
Best for: practices wanting the lowest published rates and a specific list of healthcare tasks.
Pricing: $6–$10/hrLocation: Brazil, Argentina, Mexico, Colombia, Chile, PeruModel: Recruitment placementTrial: No contract; 12-month replacement guarantee
Remote Leverage names its six talent countries outright, which is more specific than most of this region manages, and publishes the lowest rates here at $6 to $8 an hour for entry level and $8 to $10 for experienced staff, plus a one-time placement fee and a $100 refundable deposit to begin a search. Its healthcare role list is the most itemized of any firm on this list, covering patient intake and scheduling, insurance verification, prior authorization, medical billing, care coordination, EHR management, revenue cycle support, records management, and virtual medical reception. Every candidate must submit an English voice recording, and it offers no contract, no recurring fee, and a twelve-month replacement guarantee.
Limitation: no HIPAA or BAA statement appears anywhere, despite selling billing, prior authorization, and EHR work, so compliance is entirely yours to construct.
5. Pearl Talent
Best for: practices that want to compare Latin American and Filipino candidates through one provider.
Pricing: $9.95/hr, or from $3,000/moLocation: Latin America and PhilippinesModel: Managed placementTrial: Not publicly listed
Pearl Talent recruits across both Latin America and the Philippines and publishes $9.95 an hour, or from $3,000 a month for a managed arrangement, with compliance managed on its side. The dual-region model has real practical value if you haven't settled the trade-off yet, because you can trial a Latin American assistant working your daytime against a Filipino one at a lower rate without changing vendor or contract. It serves general business clients alongside healthcare.
Limitation: it serves non-healthcare clients too, so confirm healthcare depth and ask what "compliance managed" covers in practice.
6. Cloudstaff
Best for: larger practices that need documented security certifications before anything leaves the building.
Pricing: Not publicly listedLocation: Colombia, Philippines, India, KenyaModel: Managed offshore teamsTrial: Not publicly listed
Cloudstaff sources Latin American talent in Colombia alongside three other countries, and it's the strongest firm in this comparison on documented security, stating HIPAA compliance plus ISO 27001, ISO 9001, PCI-DSS, and GDPR, with a 99.99% security score from SecurityScorecard. Given how thin compliance statements are across this region, that documentation is a genuine differentiator rather than a checkbox. It reports serving over 1,000 companies with 99.5% monthly staff retention, and puts typical savings for a team of five to ten at $150,000 to $450,000 a year.
Limitation: healthcare is one vertical among many with no medical roles itemized, so clinical depth needs testing in the interview.
7. WOW Remote Teams
Best for: practices that want a short refund window while they test whether the model works.
Pricing: Not publicly listedLocation: Latin AmericaModel: Managed placementTrial: Two-week satisfaction guarantee with refund
WOW Remote Teams recruits across Latin America and lists medical and healthcare as one of its role categories. Its pricing approach is described rather than published, priced by seniority, skills, English level, and hiring model, which at least acknowledges that those are the variables. The most concrete term on offer is commercial, a two-week satisfaction guarantee with a refund if the professional isn't right, which lowers the cost of trying the region without committing.
Limitation: neither countries nor rates are published, and there's no HIPAA statement at all despite offering healthcare roles.
Latin America or the Philippines?
This is the decision most practices are making, and it comes down to three things rather than price alone.
Latin America wins on hours and on Spanish. Your assistant works days, which matters enormously for anything patient-facing, because a receptionist on a permanent night shift is a retention problem waiting to happen. Spanish comes as standard, so a bilingual front desk costs nothing extra. Published rates here run from $6 to $12.65 an hour, so the region is no longer meaningfully more expensive than Asia at the entry level.
The Philippines wins on clinical depth and on compliance maturity. The country trains far more nurses than it employs, which is why Filipino scribes so often arrive already fluent in clinical vocabulary, and the outsourcing industry there has been serving US healthcare long enough that HIPAA language and BAAs are routine. On this Latin American list, only two of seven firms make a clear HIPAA statement, and two say nothing at all. That gap is the strongest argument for the Philippines on documentation and billing roles.
The practical answer for many practices is to split by role, putting patient-facing work in Latin America for the hours and the language, and documentation or billing work in the Philippines for the clinical literacy. Three firms here recruit in both regions, so you can do that without managing two vendors.
Which HIPAA gap should you check when hiring from Latin America?
- Thin HIPAA posture. Only two firms here state it clearly, and two don't mention it at all. Assume nothing and get a BAA in writing.
- "HIPAA-conscious" or "compliance managed" language that sounds like a commitment and isn't. Ask what specifically is signed.
- English variation across a large region. Ask how English is screened; Remote Leverage requires a voice recording, which is the right idea.
- Recruitment models that leave training, supervision, and compliance design with you.
- Countries unnamed, so you can't assess local data-protection law or political stability.
- Public holidays differing by country, which quietly removes cover on days you didn't plan for.
- Clinical scope creep, above all where an assistant has healthcare background but no US license.
- Placement fees and deposits that make trying several providers more expensive than it looks.
How much does a Latin American medical virtual assistant cost?
Published rates run from $6 an hour to $12.65. Remote Leverage posts $6 to $10 depending on experience, Pearl Talent $9.95, Honest Taskers $10.00 to $12.65, and Teem $11.50. Virtual Latinos prices monthly at $1.8K to $3.7K for full-time at 160 hours, which works out roughly $11 to $23 an hour depending on tier. Cloudstaff and WOW Remote Teams quote on request.
Watch for costs outside the hourly rate, because this region uses them more than the Philippines does. Remote Leverage charges a one-time placement fee plus a $100 refundable deposit to start a search, and monthly full-time pricing looks cheaper per hour until you realise you're committing to 160 hours whether you need them or not. Set against a US front-desk or admin wage plus payroll costs, every option here is a substantial saving, and the useful comparison is total first-year cost including fees, turnover, and the hours you need rather than the hours in the package. Rates move, so treat these as dated readings.
What should a Latin America medical virtual assistant company put in writing?
- A signed BAA, in writing. This is the region's weakest area and the most important question to ask.
- A stated talent location, ideally naming countries rather than the region.
- How English is screened, and whether you hear the candidate before committing.
- Confirmation the assistant works your hours, which is the region's main advantage.
- Whether Spanish is genuinely fluent or merely conversational, if patients will rely on it.
- Placement fees, deposits, and minimum hours, not just the hourly rate.
- Replacement terms. Remote Leverage offers twelve months, WOW two weeks with a refund.
- A written clinical boundary, above all for staff with healthcare backgrounds.
Is the hours overlap or the Spanish your reason for hiring from Latin America?
- Write the task list, separating administrative work from anything clinical.
- Decide whether the hours overlap or the Spanish is the reason you're choosing this region.
- Shortlist two or three firms that state a Latin American talent base.
- Ask each one directly whether they will sign a BAA, and treat a vague answer as an answer.
- Hear the candidate speak before committing, in both languages if that matters.
- Get placement fees, deposits, and minimum hours in writing alongside the rate.
- Start with a narrow scope, then widen once it's steady.
- Review at two weeks and 60 days on coverage, accuracy, and hours your team recovered.
Methodology and sources
We verified each company at its own website on August 21, 2026, recording stated talent location, published pricing, the healthcare roles each company names, time-zone and language capability, and HIPAA posture including whether a BAA is stated. Business associate requirements come from the U.S. Department of Health and Human Services. Rates are volatile and were captured as dated points rather than quotes, so re-check before you buy. Facts a company didn't publish are marked "not publicly listed", and self-stated claims are labeled company-reported. Because these titles ask for talent from Latin America rather than a single country, firms stating LATAM as their talent base qualify even without naming countries; firms publishing no location at all were excluded. Solvo Global, NeoWork and SuperStaff were left out because their pages returned errors or truncated content, so nothing could be verified from source.
