How we chose these companies, and why there are three
Pakistan's healthcare outsourcing industry is real, substantial, and concentrated on exactly this role. Lahore and Karachi hold a genuine medical billing and revenue cycle sector serving US practices, and the talent is there. What is almost never there is a public statement of where the work happens. Open the sites of the firms a directory recommends and you find a US business address, a Pakistani phone number, and no country named. Two exclusions show why the rule earns its keep. One firm a major directory names among the top Pakistani medical billing companies sells truck dispatch and tax services on its own site. Another well-known billing name now resolves to an empty page under different branding. Both would have passed a list built from directory data, and neither should be in your procurement process.
The good news for this role specifically is that billing tolerates the time difference better than any other. Pakistan sits roughly nine to twelve hours ahead of the US mainland, and most billing work is asynchronous: claims go out, payments post, denials get worked from a queue. None of that needs a live conversation. The exception is payer phone calls, which only happen during US business hours, so agree an overlap window if your denials get resolved by voice rather than by portal. Business associate requirements come from the U.S. Department of Health and Human Services, which sets no geographic limit on where claims may be processed.

Virtual Assistants Pakistan states plainly that it is Pakistan based, which on the criterion this search depends on puts it ahead of most of the country's billing sector, and it names a medical billing virtual assistant and a virtual medical coder as separate roles rather than one blurred function. That distinction matters, because coding and billing require different training and conflating them is how practices end up with code assignment being done by someone unqualified to do it. It states 24/7 coverage and flexible shifts, with NDAs and compliance agreements signed and assistants trained in HIPAA guidelines.
Limitation: HIPAA-compliant support is described as available "on request" and no business associate agreement is stated, which for work that is entirely PHI is the wrong way round. No rate and no trial terms are published either, so cost and commitment both have to come from a conversation.
Should you choose a placed biller, a Pakistani billing firm, or a US billing company?
A placed biller works your claims inside your systems at an hourly or weekly rate and leaves the strategy and the numbers with you. At roughly $6.64 to $12.65 an hour it's the cheapest route per dollar collected once volume is meaningful, and it suits a practice whose process works and whose problem is capacity.
A Pakistani billing and revenue cycle firm is what most of the country's industry sells: a team working your claims on their own process, priced per claim or as a percentage of collections. The economics are attractive and the sector is experienced. The diligence burden is higher than elsewhere, because pricing, certifications, and delivery location are published far less often, so you'll be establishing basics that other markets put on the page. Firms that publish a per-claim rate exist, and one we reviewed quoted from $3 per claim, but it named only a US address, so it could not be listed here.
A US billing company is the third route, with software, a team, and accountability for the whole cycle at a percentage of collections. Buy staffing when you need hands, a Pakistani billing firm when you need throughput at the lowest cost and can do the diligence, and a US company when your process itself is the problem and you want someone in the same jurisdiction answerable for fixing it.

For billing, the rate is the least interesting number. Judge against collections instead: a biller at $12 an hour who works denials on their merits and appeals within the payer's own deadlines returns more than one at $7 who posts payments and closes tickets, because the gap between those two people is measured in recovered claims rather than in hours logged. Ask any candidate what they did with the last denial they couldn't clear on the first call, and listen for whether the answer cites the payer's policy or just a resubmission. Twenty to forty hours a week is the usual range, landing roughly $550 to $2,000 a month. Rates move, so confirm before you sign.

This list has three entries because three companies state Pakistan as a place their people work, out of eight checked. Bizdaar LLC publishes a Wyoming address with a Pakistani phone line and describes a "HIPAA-aware" process, Meds Nexus was founded in New York, shows a Pakistani phone code and advertises billing "as low as 2.69%", Pyramids Global publishes an Austin, Texas address with medical billing "from $3 per claim" and states HIPAA compliance, and HiredSupport states no location at all. All four fail the location test despite being capable billing firms, which is exactly the problem this category has. Medics Billing now resolves to an empty page under different branding, Medmax Technologies redirects to a domain returning an error, TriTech Services returns a server error, and ePlanet could not be resolved. REVMAVENS is excluded on substance: named by a major directory as a top Pakistani medical billing company, its own site sells truck dispatch and tax services.
