Honest Taskers, MedStar BTR, and DocVA lead this year's shortlist of companies to hire a virtual medical biller from the Philippines, seven firms ranked on whether they state that their billers are Filipino, whether those billers hold a coding credential, what they charge, and how claims work gets done overnight without your accounts receivable aging. Billing is the one role where a certification is worth paying for, so we checked which firms name one. Below we set out the choice between a Filipino biller, a billing company and software, how to check coding accuracy, what it costs, which certification the hire holds, and what is outstanding past 30, 60 and 90 days.
Disclosure: Honest Taskers publishes this list and appears at position 1. We're judged on the same five published criteria as every other company here.

How we chose these companies
Billing is the role where offshore hiring makes the clearest financial sense and where getting it wrong costs the most. A claim denied for a coding error doesn't just delay payment, it consumes staff time twice, so the credential behind the person matters more here than in reception or scheduling. We compared five things across every firm, which are stated talent location, whether a billing or coding credential is named, published pricing, the billing scope each company describes, and HIPAA posture including whether a BAA is signed.
As with every article in this series, the first filter was location. If a company doesn't say where its staff are, it can't appear on a list about hiring from the Philippines, and several capable firms were left out on that basis rather than assumed into place. What remained were companies that state a Filipino workforce, and among those the differentiator is credentialing. MedStar BTR names Certified Billing and Coding Specialist among its staff certifications, and DocVA names certified billers specifically, which is more concrete than a general claim about billing support.
The time zone works in your favor for this role, unusually. Claims work, denial follow-up, and payment posting don't need to happen while your office is open, so a Manila day shift can process yesterday's claims and hand you a cleared queue each morning. That's the opposite of the night-shift problem that affects reception hiring, and it's the strongest structural argument for offshoring billing rather than phones. HIPAA still applies in full, with the assistant also working under the Philippines Data Privacy Act of 2012, and the U.S. Department of Health and Human Services sets out what a business associate arrangement requires.

1. Honest Taskers
Best for: practices that want billing and coding as defined roles, at a published rate, with a trial before committing.
Pricing: $10.00–$12.65/hrLocation: PH, LatAm, India and Pakistan recruitingModel: Managed staffingFirst hire: Two-week working trial
Honest Taskers recruits in the Philippines, Latin America, India and Pakistan and runs virtual medical billing and medical coding as separate roles rather than folding them into a general assistant, which is what you want when the work is claims rather than admin. The scope covers claim preparation and submission, denial follow-up worked to a conclusion, payment posting, insurance verification, prior authorizations, and the patient conversations about balances that nobody at the front desk has time for, all inside your existing billing system and EHR. Staff train on HIPAA and data privacy under a dedicated compliance officer, a Business Associate Agreement is signed where they'll access PHI, and HIPAA compliance is verified by Accountable. Rates run $10.00 to $12.65 an hour, the first hire includes a two-week working trial, and the company reports 99.6% average monthly retention, which matters in billing because a biller who knows your payers is materially faster by month three. See the virtual medical billing service for how the role is structured.
Limitation: it recruits across four regions, so confirm your biller comes from the Philippines if that's specifically what you want.
2. MedStar BTR
Best for: practices that want a formally certified biller rather than an assistant who also does billing.
Pricing: Not publicly listedLocation: PhilippinesModel: Trained placementTrial: Not publicly listed
MedStar BTR is the strongest option here on credentialing, naming Certified Billing and Coding Specialist among the certifications its staff hold, and it lists biller as its own role alongside receptionist and administrative assistant rather than bundling everything together. Its people are Filipino graduates of allied and non-allied healthcare programs trained toward US administrative standards. For billing that formal grounding is the point, because the difference between a competent biller and a general assistant showing up in your claims data is measured in denial rates rather than in enthusiasm. Privacy training is required before placement.
Limitation: no rate is published, so you can't compare it against the hourly figures here without requesting a quote.
3. DocVA
Best for: practices wanting billing, collections, and prior authorization from one provider with no lock-in.
Pricing: Not publicly listedLocation: Philippines Data Privacy Act jurisdictionModel: Offshore staffingTrial: Contract with no lock-in period
DocVA names certified billers among its staff and covers the whole revenue-adjacent workload, including billing and collections support, insurance verification, prior authorization, and medical records and chart preparation, so the paperwork that surrounds a claim sits with the same provider as the claim itself. That breadth matters in billing because denials are frequently caused upstream by a missing authorization or an unverified benefit rather than by the coding. Commercially it describes a contract with no lock-in period and cancellation at any time, and frames cost as half an in-person hire or less.
Limitation: it states the Philippine privacy law it works under rather than naming the talent country, and no BAA is described, so get both confirmed in writing before any claims data moves.
4. Portiva
Best for: smaller practices wanting a clear hourly rate for part-time billing help.
Pricing: $10/hrLocation: PhilippinesModel: Dedicated assistantTrial: Not publicly listed
Portiva staffs from the Philippines at a published $10 an hour with HIPAA training for its assistants, and the value here is predictability rather than specialisation. A practice with ten or fifteen hours a week of billing follow-up can buy exactly that at a known rate, without negotiating a package or committing to a full seat. It's a Philippines-only provider, so there's no ambiguity about where the work happens.
Limitation: billing isn't itemized as a separate credentialed role, so test claims and coding knowledge specifically in the interview.
5. Cloudstaff
Best for: groups that need documented security certifications before claims data leaves the building.
Pricing: Not publicly listedLocation: Philippines, Colombia, India, KenyaModel: Managed offshore teamsTrial: Not publicly listed
Cloudstaff is the most documented provider here on data security, which carries real weight for billing because the role touches both PHI and payment information. It states HIPAA compliance alongside ISO 27001, ISO 9001, PCI-DSS, and GDPR, plus a 99.99% security score from SecurityScorecard, and PCI-DSS in particular is relevant when card details are involved. It sources talent in the Philippines, Colombia, India, and Kenya, reports serving over 1,000 companies with 99.5% monthly staff retention, and puts typical savings for a team of five to ten at $150,000 to $450,000 a year.
Limitation: healthcare is one vertical among many with no billing roles itemized, so revenue-cycle depth needs establishing rather than assumed.
6. VA MASTERS
Best for: practices wanting the lowest published rate for invoicing and balance follow-up.
Pricing: $6.50–$10.00/hrLocation: PhilippinesModel: Recruitment placementTrial: No payment until you approve a candidate
VA MASTERS publishes the lowest range here at $6.50 to $10.00 an hour, roughly $1,040 to $1,600 a month at 160 hours, and describes its Filipino assistants as covering practice-administration work including invoicing. For chasing balances and posting payments, where the value is persistence rather than coding expertise, that rate is hard to argue with. It runs a six-stage vetting process with no upfront recruitment fee and no payment until you approve the candidate, so a poor match costs time rather than money.
Limitation: a recruitment model that puts data-handling policy on the practice, which is a heavier responsibility when payment information is involved.
7. Pearl Talent
Best for: practices that want the option of Filipino or Latin American billers from one provider.
Pricing: $9.95/hr, or from $3,000/moLocation: Latin America and PhilippinesModel: Managed placementTrial: Not publicly listed
Pearl Talent recruits across both Latin America and the Philippines and publishes $9.95 an hour, or from $3,000 a month for a managed arrangement, with compliance managed on its side. The dual-region model is the reason to consider it for billing, because it lets you compare a Filipino biller working your overnight against a Latin American one working your business hours without changing vendor. For a practice that hasn't decided which pattern suits its revenue cycle, that flexibility has value.
Limitation: it serves non-healthcare clients too and doesn't name a billing credential, so confirm both healthcare depth and coding knowledge.
Should you choose a Filipino biller, a billing company, or software?
Three routes compete and they suit different practices. Hiring an offshore biller gives you a person working your systems at a fraction of a US wage, and you keep full visibility of the claims. It works when you have someone internally who can supervise the work and answer payer-specific questions.
A a billing company that takes the whole revenue cycle and typically charges a percentage of collections, which aligns their incentive with yours and removes the management burden entirely. It costs more when collections are healthy, and it means giving up day-to-day visibility. For a practice with nobody able to supervise a biller, that trade is usually correct.
Billing software fixes a different problem, which is claim scrubbing, eligibility checking, and workflow. It reduces errors before submission and it doesn't chase anything. Software plus an offshore biller is a common and sensible combination, since the tool catches the predictable errors and the person handles the denials that need a phone call. What rarely works is buying software and expecting existing front-desk staff to absorb the follow-up, which is how accounts receivable quietly ages past ninety days.
How do you check a Filipino medical biller’s coding accuracy?
- Ask for a denial rate and for how coding accuracy is checked, because coding errors are what drive denials here. Ask for a denial rate and how coding accuracy is checked.
- No BAA in place, which matters more here because the role touches both PHI and payment data.
- Payment card handling without PCI discipline. Ask specifically how card details are captured and stored.
- No segregation of duties, where one person bills, posts, and reconciles unchecked.
- Payer-specific knowledge assumed rather than tested. US payer rules don't transfer from general billing experience.
- Location claims you can't verify, since a US business address is not a talent location.
- Credentials asserted loosely. Ask which certification, from which body, and when it was obtained.
- Time-zone gaps on payer phone calls, since US insurers answer during US hours.
How much does a Filipino medical biller cost?
Published rates run from $6.50 an hour to $12.65. VA MASTERS posts $6.50 to $10.00, Pearl Talent $9.95, Portiva $10, and Honest Taskers $10.00 to $12.65. MedStar BTR, DocVA, and Cloudstaff quote on request. Against a US biller's fully loaded cost, all of these represent a substantial reduction, which is why billing is usually the first role a practice offshores.
The right comparison isn't rate against rate though, it's rate against recovery. Pull your aging report and total what sits past ninety days, then ask what a biller at twenty hours a week might realistically recover from it. At these rates that's roughly $500 to $1,000 a month of cost, and most practices have considerably more than that sitting uncollected. Credentialed billers cost more and generally pay for themselves through lower denial rates, which is the one place in this market where paying up is usually the cheaper decision. Rates move, so confirm before you commit.
Which certification does the billing hire hold, and who issued it?
- Ask for the body by name rather than a general claim about billing experience, with the talent location stated.
- A stated talent location in the company's own words.
- A signed BAA, plus how payment data specifically is handled.
- Experience with your payer mix, tested rather than assumed.
- Experience in your billing system, named.
- Overlap with US payer phone hours, since some denials only resolve by phone.
- A weekly aging report you receive.
- Segregation of duties between billing, posting, and reconciliation.
What is outstanding past 30, 60 and 90 days before a Filipino biller starts?
- Pull your aging report and total what's outstanding past 30, 60, and 90 days. That's your baseline.
- Decide whether you need coding expertise or persistence on follow-up, because they price differently.
- Shortlist two or three firms that state a Philippines talent location.
- Ask for certifications by name and issuing body.
- Get the BAA and the payment-data handling process in writing.
- Test the candidate on a real denial scenario from your own payer mix.
- Start with chasing existing balances, which shows a return fastest.
- Review at 60 days against your opening aging figures and denial rate.
Methodology and sources
We verified each company at its own website on August 21, 2026, recording stated talent location, whether a billing or coding credential is named, published pricing, the billing scope described, and HIPAA posture including whether a BAA is stated. Business associate requirements come from the U.S. Department of Health and Human Services. Rates are volatile and were captured as dated points rather than quotes, so re-check before you buy. Facts a company didn't publish are marked "not publicly listed", and self-stated claims are labeled company-reported. Companies that do not state a talent location were excluded even where they are strong billing providers, because a country-specific comparison cannot rest on an assumed location. DocVA is included on the basis of its stated operation under the Philippines Data Privacy Act of 2012, with that reasoning noted in its entry rather than presented as a plain claim.
