Q: Which firm specializes in post-discharge follow-up?

RelateCare is the only one here that names post-discharge follow-up as a service in its own right, paired with telephonic nurse triage giving patients 24/7 access to healthcare advice. Everyone else reaches recently discharged patients through a care management program rather than a discharge service.

Q: What causes readmissions that coordination can prevent?

Logistics more often than clinical decline: a follow-up appointment nobody booked, a home health referral never accepted, equipment that did not arrive, a prescription not collected, and records that never reached the receiving clinician. All administrative, all deadline-driven.

Q: How is discharge coordination priced?

Honest Taskers publishes $10.00 to $12.65 an hour, covering every discharge rather than enrolled patients only. Signallamp Health states $0 out of pocket on a revenue share and CareHarmony states $0 upfront investment. RelateCare, Carenet Healthcare, ChartSpan, Wellbox, SimiTree and Richter quote on request.

How soon after discharge should the follow-up call happen?

Within days, not weeks. The value collapses quickly: an early call catches medication confusion and the unbooked follow-up appointment, while the same call three weeks later functions as a satisfaction survey. Ask for the target interval and confirm weekend coverage, since discharges cluster before weekends.

Can an administrative coordinator do medication reconciliation?

No, reconciliation is licensed clinical work and belongs with a nurse or pharmacist. Signallamp Health names it as a service, delivered by its own US-based RNs and LPNs licensed in the patient's state. An administrative coordinator can confirm the patient collected their prescriptions, which is a different task.

Why are SimiTree and Richter on a discharge list?

Because discharge has a second meaning. Both handle the documentation consequences rather than the patient: SimiTree performs discharge reviews within its coding and documentation review service, and Richter provides additional documentation request management and audit support. If your discharge problem is denials rather than readmissions, they are the right answer.

Honest Taskers, RelateCare, and Carenet Healthcare lead this year's shortlist of virtual discharge coordinator companies, nine firms ranked on how they handle the transition out of a hospital or facility, from arranging follow-up appointments and home health referrals before the patient leaves to the post-discharge call, medication reconciliation, symptom check, and the readmission that all of it exists to prevent. The split on this list is between administrative transition work you can staff hourly and clinical post-discharge nursing paid for out of care management reimbursement.

Disclosure: Honest Taskers publishes this list and appears at position 1. We're judged on the same five published criteria as every other company here.

How we chose the best virtual discharge coordinator companies

We ranked these firms on five facts, each read at the provider's own website in August 2026. Those facts are whether post-discharge work is named or implied, whether the caller is clinical or administrative, published pricing, the fee model, and named compliance. The timing of the contact is what we weighted hardest. Post-discharge value collapses quickly: a call in the first few days catches the medication confusion and the missed follow-up appointment, while the same call three weeks later is a survey. So a firm that names post-discharge follow-up as a service, with hours that cover the days after a weekend discharge, is doing something different from a firm that reaches the same patient on a monthly care management cycle. Blank facts read "not publicly listed" instead of a guess.

Best virtual discharge coordinator companies compared

The table ranks all 9 on the same columns, Honest Taskers first because we publish the list. Facts were read August 2026 and change often, so treat them as a starting point, not a quote.

Virtual discharge coordinator companies compared on the criteria stated below, read at each provider's own site.
Company	Discharge work covered	Who makes contact	Published pricing	Fee model	Compliance named
Honest Taskers	Follow-up appointment booking, home health and equipment referrals, records transfer, post-discharge check calls, rebooking, insurance and authorization checks	Placed administrative staff in your systems; pool includes licensed nurses and physicians	$10.00–$12.65/hr	Staffing by the hour	Compliance officer; HIPAA compliance verified by Accountable; BAA when PHI is accessed
RelateCare	Post-discharge follow-up named, telephonic nurse triage with 24/7 access to healthcare advice, wellness programs	Nurses on a 24/7 triage line	Not publicly listed	Managed service, enterprise	Not publicly listed
Carenet Healthcare	Nurse triage, after-hours clinical support, physician e-consults, patient engagement at scale	Registered nurses plus engagement staff	Not publicly listed	Managed service at scale	Not publicly listed
CareHarmony	Nurse-led care coordination with engagement specialists and population health targeting	Nurses, engagement specialists, data scientists	$0 upfront investment	Managed program plus CareBlocks platform	Not publicly listed
Signallamp Health	Care management, medication reconciliation, care planning, remote monitoring inside your EHR	Its own home-based RNs and LPNs, 100% US-based, licensed in the same state as your patients	$0 out of pocket	Revenue share against existing CMS codes	Not publicly listed
ChartSpan	Chronic care enrollment and monthly outreach, 24/7 nurse care line	Its own clinical staff	Not publicly listed	Managed program, whole function	Not publicly listed
Wellbox	Monthly nurse interactions, risk identification, preventative care coordination	Nurses; credentials not published	Not publicly listed	Managed program	Not publicly listed
SimiTree	Discharge reviews within post-acute coding and documentation review; RCM and billing alongside	Coding and review staff, not patient-facing	Not publicly listed	Outsourced services; 48-hour coding turnaround	Not publicly listed on the pages read
Richter	ADR management, TPE audit support, QAPI, outsourced revenue cycle across SNF, home health, hospice and senior living	Consultants, not patient-facing	Not publicly listed	Outsourced functions and consulting	Not publicly listed

Only one firm names post-discharge follow-up as its own service. Two entries at the bottom are honest inclusions rather than patient-facing options: they review discharge documentation and defend the claim, which is a real part of discharge work and a different product entirely. Note also that not one firm on this list names an audited security certification, which is worth pressing on for a category where clinical detail moves between organizations.

1. Honest Taskers

Best for: practices and facilities that need the transition logistics owned by name, for every discharged patient rather than a qualifying subset.

Pricing: $10.00–$12.65/hrLocation: US clients; PH, LatAm, India and Pakistan recruitingModel: Staffing by the hourFirst hire: Two-week working trial

Most readmissions do not begin with a clinical failure. They begin with a follow-up appointment nobody booked, a home health referral that never got accepted, equipment that did not arrive, a prescription the patient could not collect, and records that never reached the receiving clinician. Honest Taskers places staff who work inside your systems on that whole chain: booking the follow-up visit before the patient leaves, placing and confirming home health and durable medical equipment referrals, checking insurance and authorization for what has been ordered, transferring records to the receiving provider, calling the patient in the first days to confirm the appointment and that the medications were obtained, and rebooking immediately if the appointment is missed. Because it is hourly, it covers every discharge rather than the patients who qualify for a billable program. Staff are trained on HIPAA and data privacy under a dedicated compliance officer, work in your time zone, and a Business Associate Agreement is signed before anyone touches protected health information, with HIPAA compliance verified by Accountable. The talent pool includes licensed nurses and physicians. Rates run about $10 to $12.65 an hour by role, background, schedule, and location, with a two-week working trial on the first hire and 99.6% average monthly retention reported. See the multi-purpose virtual medical assistant service for role detail.

Limitation: staff work remotely and administratively, so symptom assessment, medication reconciliation, and clinical advice stay with your licensed clinicians, and there is no 24-hour nurse triage line attached.

2. RelateCare

Best for: hospitals and groups whose readmission exposure needs a nursing service built specifically for the post-discharge window.

Pricing: Not publicly listedLocation: Not publicly listedModel: Managed service, enterpriseFocus: Post-discharge follow-up

RelateCare is the only firm on this list that names post-discharge follow-up as a service in its own right rather than as something its care management program happens to touch, and in this category that specificity is the whole point. It pairs that follow-up with telephonic nurse triage giving patients 24/7 access to healthcare advice, which is the combination that works: the follow-up call surfaces a problem and the triage line handles it without waiting for office hours. Wellness programs extend contact beyond the immediate window. For an organization measured on thirty-day readmissions, this is the closest match on the list to the problem as stated.

Limitation: it is enterprise-oriented with no published pricing and no named compliance certification, so small-practice fit and the security position both need establishing before you invest time.

3. Carenet Healthcare

Best for: organizations that need discharge follow-up and the inbound calls it generates handled in one operation.

Pricing: Not publicly listedLocation: Not publicly listedModel: Managed service at scaleFocus: Triage and engagement

Carenet Healthcare's strength for discharge work is that outbound and inbound sit in the same team. A post-discharge call produces a callback, and a callback about a wound or a breathing problem needs a nurse rather than a message taker. Carenet combines patient-engagement staff with registered nurses and physician e-consults into a single triaged service, and names after-hours clinical support explicitly, which matters because discharges cluster before weekends. It reports a 67% cost reduction, a company-reported figure. The volume-based model suits health systems, groups, and health plans rather than small practices.

Limitation: post-discharge follow-up is not a named service line, the administrative transition work is out of scope, and it publishes neither pricing nor any compliance certification.

4. CareHarmony

Best for: practices that want follow-up effort concentrated on the discharged patients most likely to bounce back.

Pricing: $0 upfront investmentLocation: HQ Brentwood, TennesseeModel: Managed program plus platformScale: 100,000+ people (company-reported)

CareHarmony brings targeting to a problem that usually gets treated as a queue. Its nurses work alongside population-health experts, data scientists, and engagement specialists on the CareBlocks platform, and identifying which discharged patients carry the highest readmission risk is exactly the kind of question that layer answers. When you have sixty discharges a week and capacity for twenty calls, choosing the right twenty is worth more than making all sixty badly. Commercially it states $0 upfront investment, so the work is funded from program revenue rather than your budget, and the company reports more than 100,000 people relying on it from Brentwood, Tennessee.

Limitation: discharge follow-up is folded into general care coordination for enrolled patients, and the site names no staff credentials, no pricing detail, and no HIPAA or Business Associate Agreement statement.

5. Signallamp Health

Best for: practices that want medication reconciliation after discharge done by a nurse licensed in the patient's state.

Pricing: $0 out of pocketLocation: 100% US-basedModel: Revenue share on CMS codesLaunch: 4 to 6 weeks

Signallamp Health names medication reconciliation as a service, and after a hospital stay that is the highest-value clinical task there is: the discharge summary and the patient's actual medicine cabinet rarely agree, and the gap between them is where readmissions come from. It employs its own home-based registered nurses and licensed practical nurses, states they are entirely US-based and licensed in the same state as your patients, and works directly in your electronic health record, so the reconciled list lands where your clinicians will read it rather than in a vendor portal. Care planning and remote monitoring sit alongside. The model is a revenue share against existing CMS codes, described as $0 out of pocket, with a four to six week launch, and it reports 86% improvement in medication adherence and 63% lower emergency department use.

Limitation: the work reaches care management patients rather than everyone discharged, the site states no HIPAA or BAA position, and the revenue share needs billable volume to make sense.

6. ChartSpan

Best for: practices that want discharged chronic patients absorbed into a standing monthly program with a nurse line.

Pricing: Not publicly listedLocation: Not publicly listedModel: Managed program, whole functionExtras: 24/7 nurse care line

ChartSpan is the option for the patient after the immediate window closes. Its clinical staff run monthly chronic care outreach with time tracking and documentation, it handles the enrollment conversations, and the 24/7 nurse care line gives a recently discharged patient somewhere to call at 10pm instead of returning to an emergency department. That last piece is the part that matters most for readmissions, and it is available to every enrolled patient rather than only for a defined number of days. Company-reported outcomes are $2,457 in annual patient savings and 20% fewer hospitalisations.

Limitation: there is no post-discharge service line and no coverage of the transition logistics, so the first critical days after discharge are not what this is built for.

7. Wellbox

Best for: practices wanting steady nurse contact that catches deterioration in the weeks after a stay.

Pricing: Not publicly listedLocation: Not publicly listedModel: Managed programFocus: Monthly nurse interactions

Wellbox works on a monthly nurse interaction covering risk identification, chronic care management, patient engagement, and preventative care coordination, and the continuity is genuine: the nurse who spoke to the patient last month notices what has changed since the hospital stay. Company-reported outcomes are the strongest claims on this list, with emergency room visits and hospitalisations down 60%, per-beneficiary-per-month Medicare costs down 10% to 15%, and patient satisfaction between 90% and 96%.

Limitation: a monthly cadence cannot cover the post-discharge window where readmission risk concentrates, and the site names no staff credentials, no pricing, and no HIPAA or BAA position.

8. SimiTree

Best for: post-acute agencies whose discharge problem is documentation and claim defensibility rather than patient contact.

Pricing: Not publicly listedLocation: Hamden, ConnecticutModel: Outsourced services and consultingScale: 750+ active clients

SimiTree is here for a different reading of the word discharge, and it is a legitimate one. It performs discharge reviews as part of its coding and documentation review service across home health, hospice, and behavioral health, alongside revenue cycle management, billing, compliance and regulatory risk work, cost reporting, credentialing, and data analysis. For an agency where discharges are triggering claim problems or audit attention rather than readmissions, this is the relevant product. Coding carries a stated 98% accuracy rate and a 48-hour standard turnaround, with company-reported scale of 1,200 or more experts and 750 or more active clients.

Limitation: nobody at SimiTree calls your patients, so it does nothing for readmission risk directly, and it publishes no pricing, no coder credentials, and no compliance certification.

9. Richter

Best for: multi-setting operators whose discharge documentation is drawing audit and additional documentation requests.

Pricing: Not publicly listedLocation: Twinsburg, OhioModel: Outsourced functions and consultingScale: 90+ professionals

Richter takes the same non-clinical reading of discharge and adds the audit dimension. It provides additional documentation request management and targeted probe and educate audit support, QAPI consulting, outsourced revenue cycle management, and accounting and controller services, across skilled nursing and long-term care, home health, hospice, and senior living. For an operator moving patients between its own settings, having one firm that understands the documentation each transition requires is worth real money when a reviewer starts asking. Named staff hold RN, RAC-CT, CPA, and CGMA credentials across more than 90 professionals.

Limitation: it is entirely non-patient-facing, so it addresses the paperwork consequences of discharge rather than the patient's transition, and it publishes no pricing or compliance position.

Should discharge work be staffed with a coordinator or handed to a clinical service?

Staff the logistics, because that is where most readmissions start. Booking the follow-up appointment, placing and confirming referrals, checking authorization for ordered equipment, transferring records, and calling to confirm the patient has their medications are all administrative tasks with tight deadlines, and hourly remote staff cover every discharge rather than a qualifying subset. A medical records specialist earned about $51,140 a year, roughly $24.59 an hour before benefits, according to the U.S. Bureau of Labor Statistics May 2025 wage data for medical records specialists, against roughly $10 to $13 an hour for placed remote staff. Hand the clinical contact to a nursing service when what you need is symptom assessment, medication reconciliation, or a triage line, and remember that reconciliation in particular is licensed work that no coordinator should attempt. And if your discharge problem is claim denials and audit requests rather than patients returning, the last two entries on this list are the correct answer and the first seven are not. For the administrative scope, see the virtual medical assistant duties breakdown.

What are the risks of outsourcing discharge coordination?

Discharge is a handoff between organizations, which is precisely where accountability tends to disappear.

Who owns the patient between organizations. Write down which party is responsible for the follow-up until it is confirmed booked and attended.
Clinical questions on an administrative call. A post-discharge call invites symptom questions by design, so define the escalation path and forbid clinical reassurance.
Medication reconciliation is licensed work. Keep it with a nurse or pharmacist, and confirm the license and its jurisdiction.
Timing against weekends. Discharges cluster before weekends, so confirm the coverage schedule rather than the promised interval.
Compliance evidence. Not one firm here names an audited security certification. Get the Business Associate Agreement in writing before any handoff.

How is discharge coordination priced?

One hourly rate, two reimbursement-funded models, and six quotes. Honest Taskers publishes $10.00 to $12.65 an hour depending on role, background, schedule, and location, which covers every discharge rather than enrolled patients only. Signallamp Health states $0 out of pocket on a revenue share against existing CMS codes and CareHarmony states $0 upfront investment, both drawing the fee from care management reimbursement. RelateCare, Carenet Healthcare, ChartSpan, Wellbox, SimiTree, and Richter are quote-only, and nurse triage in this market is commonly priced per call or per member per month, though none publishes a figure. The comparison that matters is cost per discharge covered against your readmission cost, since a single avoided readmission usually funds a great deal of coordination. Rates were checked August 2026 and change often. For staffing rate context, see how much a virtual medical assistant costs in 2026.

What should you look for in a discharge coordinator partner?

Named post-discharge capability, coverage timing, escalation, and whether every discharge is covered.

Post-discharge named as a service. One firm here names it. Ask the rest how many days after discharge their first contact happens.
Coverage that includes weekends. Get the schedule, since Friday discharges are the risky ones.
Whole population or enrolled only. Establish whether the service reaches every discharged patient or only program enrollees.
An escalation path to a clinician. Ask how a concerning answer travels and how fast.
Documentation in your record. Prefer a provider working in your chart so the follow-up is visible to the next clinician.

How do you set up discharge coordinator coverage?

Start by finding out which step in the transition is failing, because the answer decides which half of this list applies.

Audit thirty discharges: was a follow-up booked, was it attended, were referrals accepted, did equipment arrive, were medications collected.
If the failures are logistical, staff the transition work and set a same-week contact standard.
If the failures are clinical, shortlist nursing services on licensure and coverage hours.
Get the Business Associate Agreement and agree the escalation path and documentation location in writing.
Re-audit after a quarter on follow-up attendance and readmission rate, then expand what worked.

Methodology and sources

We read each company's own website in August 2026, recording whether post-discharge work is named, whether callers are clinical or administrative, published pricing, fee model, and named compliance. Wage context is from the U.S. Bureau of Labor Statistics May 2025 Occupational Employment and Wage Statistics for medical records specialists. Transitional care and care management billing rules are set by the Centers for Medicare and Medicaid Services, and licensure context is the Nurse Licensure Compact administered by the National Council of State Boards of Nursing. Where a company didn't publish a fact, we marked it "not publicly listed"; adherence, utilization and savings figures are labeled company-reported and were not independently audited.

If the audit shows follow-up appointments unbooked or referrals never accepted, the fix is an owner for the transition rather than a clinical program. Our guide to tasks to outsource to a virtual medical assistant covers appointment booking, referral placement, records transfer, and confirmation calls, and where the boundary sits before a question becomes clinical.
