Q: Does moving check-out off the counter improve collections?

Yes, in most practices it does, because a dedicated coordinator works balances the front desk drops when the exit line backs up. Point-of-service collection is the cheapest money a practice ever recovers, and a remote seat that owns the balance queue chases it before it ages into a statement or a write-off.

What does a virtual patient check-out coordinator do?

A virtual patient check-out coordinator closes the visit from inside your systems after the patient leaves the room. That means booking the next appointment and setting recall, collecting copays and working down outstanding balances, sending the after-visit summary, and following up on discharge instructions. The cash drawer and the paper handoff stay at the counter.

Can a virtual check-out coordinator take a copay at the counter?

No, a cash payment handed across the counter stays with on-site staff. A remote coordinator can run a card on file, send a pay-by-text link, set up a payment plan, and post the payment, but the physical drawer and any cash a patient hands over need a person standing there.

How much does a virtual patient check-out coordinator cost compared with in-house staff?

A virtual check-out coordinator bills $10.00 to $12.65 an hour with no employer load, about $20,800 to $26,312 a year at full time. An in-house administrative hire at the $45,930 median runs about $68,252 all-in once benefits and payroll taxes are added. Run the math on your own local wages.

Do check-out coordinators need a Business Associate Agreement?

Yes, a remote coordinator who touches scheduling, balances, or after-visit summaries handles protected health information, so a signed Business Associate Agreement is required. Check-out also mixes health data with payment data, so keep system access under practice control and scope it to the minimum the role needs.

What does a virtual check-out coordinator do?

A virtual check-out coordinator closes the visit: booking the follow-up appointment, collecting or arranging payment of the balance, placing referral and imaging orders and confirming receipt, sending the visit summary and after-care instructions, and reconciling the charge against what was documented.

Can a remote coordinator take payments?

Sometimes, but ask about PCI-DSS rather than HIPAA, because card data sits under a different regime. None of the seven firms here publishes a PCI position, so get it in writing. Many practices keep card-present payments at the window and use remote staff for balance calls and payment plans instead.

How much does check-out coordination cost?

Four firms publish rates: Staffingly at $399 a week for 45 hours (under $9 an hour at ten or more people), Hello Rache at $9.50 an hour, Honest Taskers at $10.00 to $12.65 an hour, and HelpSquad Health at $8 to $13 depending on whether the work is back-office or patient-facing.

Does moving check-out remotely improve collections?

It usually improves the parts that were being skipped. Point-of-service collection at the window still needs someone present, but balance follow-up calls, payment plan setup and statement chasing are exactly the tasks a busy desk drops, and those are measurable within a month.

Who is responsible if a charge is wrong?

Your practice. A coordinator can flag that a charge does not match the documented encounter, but changing a charge is a billing decision that needs somebody with authority and, where coding is involved, appropriate credentials. Write that boundary down before starting.

Do check-out coordinators need a Business Associate Agreement?

Yes, they handle the visit record, diagnoses and balances. Honest Taskers, My Mountain Mover and Staffingly state a signed BAA, and HelpSquad Health includes one in its rate. MEDVA, DocVA and Hello Rache describe HIPAA training instead.

How fast can a referral be confirmed as received?

Ask the provider, because placing an order and confirming receipt are different tasks and only the second one prevents a lost referral. A reasonable standard is confirmation within two business days, with an escalation if the receiving office has not acknowledged it.

Can one person handle both check-in and check-out?

Yes, and many practices do it, but the same trade-off applies as with intake: when the desk is busy, closing the visit loses to greeting the next patient. If your follow-up booking rate is poor, a dedicated owner for check-out is usually the reason it improves.

Deciding between a virtual patient check-out coordinator and in-house staff is a work-allocation call before it's a budget one, so the honest place to start is where a virtual patient check-out coordinator fits against the people at your exit counter. From there it's worth naming which closing steps a patient still finishes in person, because that's the limit of the remote model. Then the list opens up: the post-visit follow-up a coordinator handles after a patient leaves, how the coordinator books the next appointment and recall on the way out, and how it collects copays and works down patient balances. Cost follows scope rather than the other way round, so this compares what an in-house check-out clerk adds to payroll each year against what an hourly virtual check-out coordinator runs a practice instead. After that come the practical questions, starting with which option keeps the exit line moving through a full clinic day, then what a skipped recall or unpaid balance costs the practice later, whether you should staff check-out at the counter or move it to a coordinator, and when a virtual check-out coordinator is worth adding beside counter staff. The wage sources behind every figure are named at the end.

Where does a virtual patient check-out coordinator fit against staff at the exit counter?

A virtual patient check-out coordinator fits wherever the closing work lives in software rather than at the physical counter. The coordinator is a healthcare-trained remote professional who picks up the post-visit queue inside your existing systems, while in-house staff own the exit counter itself, the drawer, and the face-to-face goodbye. What separates them isn't skill, it's presence. Anything a patient completes standing at the desk stays with on-site staff, and everything that happens after they walk out can move to a remote seat. So the visit closes in two places now. The counter handles the handoff a person has to be there for, and the coordinator handles booking, balances, summaries, and follow-up from inside the same practice management system. That split is what the rest of this comparison prices, because it's the thing that decides both what moves and what it costs.

Which check-out steps does a patient still finish in person at the counter?

Some check-out steps only close with a person at the counter, and that's the honest limit of the remote model, stated before any cost figure. A virtual patient check-out coordinator can't do the following, and they stay with in-house staff.

Take a cash payment a patient hands across the counter and manage the physical drawer.
Hand over printed paperwork, a paper after-visit summary, a work note, or a specimen cup.
Give the face-to-face goodbye, walk a patient to the door, or read body language for confusion.
Collect a signature on a paper form or return an insurance card left at the desk.
Answer a clinical question at the window, which stays with your licensed providers wherever they sit.

Where most of your closing work sits on that list, the comparison is already settled and you're keeping it at the counter. Read on where a meaningful share of check-out is the after-the-fact work: booking, chasing balances, sending summaries, and following up. In most practices that share is large, because the front desk absorbs it by default. Whoever is standing there when the queue builds ends up doing the recall calls and the balance follow-up between patients, so it gets squeezed rather than owned.

What post-visit follow-up does a virtual check-out coordinator handle after a patient leaves?

A virtual check-out coordinator handles the closing work that starts the moment a patient leaves the room. That's the queue a busy counter tends to defer, and it's the part that lives entirely in your systems. A coordinator, much like a virtual medical assistant working any admin queue, can own the full list from a remote seat.

Booking the next appointment and setting the recall date before the record closes.
Sending the after-visit summary through the patient portal and confirming it landed.
Following up on discharge instructions and confirming the patient understood the plan.
Collecting the copay owed and working outstanding balances down before they age.
Placing referral and imaging orders that came out of the visit and confirming receipt.

One boundary belongs in the role description. Discharge follow-up means confirming the patient has the written plan and helping them act on it, never deciding what the plan should be or giving clinical advice. That call stays with the provider. Write the line into the role rather than settling it during onboarding, and the coordinator has plenty of closing work that never needed the counter.

How does a virtual check-out coordinator book the next appointment and recall on the way out?

A virtual check-out coordinator books the next appointment by working the schedule while the visit is still fresh, before the patient's intent to return fades. Working from the provider's plan in the note, the coordinator offers the slot, confirms it, and sets the recall date so the record doesn't close with an open loop. When a patient can't commit on the spot, the coordinator drops them into a recall list and follows up by call, text, or portal message on the schedule the provider ordered. That's the difference between a recall that's owned and one that's hoped for. Picture a counter that's four patients deep, where it books the easy return, lets the hard one walk, and the recall list quietly grows. One dedicated seat runs the list to zero. For how outside firms staff this exact function, see our roundup of the best virtual patient check-out coordinator companies.

How does a virtual check-out coordinator collect copays and work down patient balances?

A virtual check-out coordinator collects copays through the tools already in the practice management system, such as a card on file, a pay-by-text link, or a payment plan, then posts the payment against the right visit. Cash a patient hands over the counter still needs a person there, but most of what a practice is owed doesn't arrive as cash. It sits as an outstanding balance the front desk never gets back to. The coordinator owns that balance queue: verifying what the patient owes after the claim adjudicates, sending the statement, taking the payment, and setting a plan for the ones who need one. Point-of-service collection is the cheapest money a practice recovers, because chasing it after it ages into a statement cycle costs staff time and collects less. One remote seat that does nothing but close balances catches the ones a busy counter writes off by default, and it reconciles each payment against what was documented so the books stay clean.

What does an in-house check-out clerk add to practice payroll each year?

An in-house check-out clerk adds far more than the salary line, because roughly a third of the seat's cost sits on top of wages. US medical secretaries and administrative assistants earned a median $45,930 a year (Source: Bureau of Labor Statistics, "Occupational Employment and Wage Statistics", May 2025, occupation code 43-6013). Receptionist-level check-out roles run lower at a $38,010 median, so the table below uses the administrative-assistant figure as the honest middle for a clerk who books, collects, and follows up. The employer load is broken out as separate components so nothing gets counted twice (Source: Bureau of Labor Statistics, "Employer Costs for Employee Compensation", March 2026).

What one in-house check-out clerk costs a US practice per year at the national median administrative wage.
Cost line	What it covers	On top of wages	Per year
Base salary	The advertised pay for the role	n/a	$45,930
Insurance	Health and related coverage	17.5%	$8,038
Paid leave	Vacation, sick days and holidays	11.9%	$5,466
Legally required	Employer FICA, unemployment, workers' compensation	10.2%	$4,685
Supplemental pay	Overtime, bonuses and shift differentials	4.5%	$2,067
Retirement and savings	Employer contributions and match	4.5%	$2,067
All-in recurring	What the seat costs before equipment or space	48.7%	about $68,252

That covers recurring cost only. Filling the seat costs an average $5,475 per hire for non-executive roles (Source: SHRM, "2025 Benchmarking Report"), and that lands again on every turnover. Coverage is the risk the table hides: one clerk is a single point of failure, so when they're out the exit line backs up and recall stops.

What does an hourly virtual check-out coordinator run a practice instead?

An hourly virtual check-out coordinator runs $10.00 to $12.65 an hour depending on role, background, schedule, and location, billed by the hour with no employer load. At 40 hours a week that's about $20,800 to $26,312 a year, and at 20 hours a week about $10,400 to $13,156. None of the load in the table above applies. There's no payroll tax, no benefits, no paid leave, and no workspace, because you're buying hours rather than employing a person. Most practices underweight the part-time figure, and check-out is often a part-time workload, since the closing queue peaks after visits rather than filling a steady eight hours. An in-house clerk is usually a full-time decision anyway, so for a genuinely part-time load the comparison isn't $68,252 against $26,312, it's $68,252 against $13,156 for the same closing work. For the pricing detail on the model, see our guide to how much a virtual medical assistant costs. Run it on your own local wages rather than these national medians.

Which check-out option keeps the exit line moving through a full clinic day?

A split setup keeps the exit line moving best through a full clinic day, because it puts the right work in the right place. The counter clears the handoff a patient has to stand there for, and the coordinator absorbs everything that would otherwise stack up behind that patient: the booking, the balance, the summary. When one clerk does both, the line stops every time a booking runs long or a balance needs a payment plan, and the patients behind wait. A remote seat working the post-visit queue in parallel means the counter person only does the face-to-face part, so throughput at the window goes up without adding a second body to the desk. The failure mode to watch is moving a whole role instead of the queue. Pull the person off the counter entirely and the handoff has nobody, so the line stops for a different reason. Move the after-visit work and keep the counter staffed, and both halves run at once.

What does a skipped recall or unpaid balance cost the practice later?

A skipped recall or unpaid balance costs the practice real revenue it has already earned, and the loss shows up weeks later where it's hard to trace back to check-out. Take the recall that never gets booked, which is a visit that doesn't happen, so the schedule shows an empty slot and nobody connects it to the closing step that got skipped. An unpaid balance ages into a statement cycle, then into a write-off, and every week it sits there it collects less. The counter doesn't skip these on purpose. They get squeezed out when the exit line is long, so the easy close happens and the hard one slips. That's the case for giving the work an owner. A dedicated coordinator runs the recall list to zero and works balances before they age, which is the point of moving check-out off a counter that's already full. Nobody budgets for the revenue that quietly walks out the door, so price it before you decide the counter can absorb the work.

Should a practice staff check-out at the counter or move it to a virtual coordinator?

Sort your closing work into two columns before you price anything, because the split decides the answer more than any rate does. In the first column put every step a patient finishes standing at the counter. Everything that happens after they leave goes in the second. Then run four tests against the columns, in this order, because each one can settle it on its own.

How big is the counter column? Where the physical handoff and the cash drawer are most of the role, keep it in-house and stop.
Does the after-visit column fill a full week? Where it doesn't, an hourly coordinator fits a workload no full-time clerk can be sized to.
What's leaking now? Where recall and balances are the thing slipping, a dedicated remote seat is aimed straight at it.
What breaks when the person is out? A single counter clerk is a single point of failure, and paid leave is in the cost table for a reason.

Most answers land as both rather than one. Where you're unsure the closing workload justifies a hire at all, our guide to the signs your practice needs a virtual assistant helps size it first.

When is a virtual check-out coordinator worth adding beside counter staff?

A virtual check-out coordinator is worth adding beside counter staff the moment your desk is deferring the after-visit work to keep the line moving. That's augmentation rather than replacement, and it shows up first as your front desk getting their day back. The counter keeps the handoff, the drawer, and the goodbye, and the coordinator takes booking, recall, balances, summaries, and discharge follow-up from a remote seat in your time zone. Weighing providers before you commit is worth the time, and our roundup of the best virtual patient check-out coordinator companies lays out how firms staff the role. Timing matters here. A remote coordinator can start fast, since most Honest Taskers placements complete within one to three weeks of a signed agreement, and the first hire comes with a two-week working trial so the fit is tested before more is committed. Honest Taskers also reports 99.6% average monthly retention, and where a placement doesn't fit the replacement runs through the same process rather than a fresh recruitment cycle. A remote coordinator works within a HIPAA-compliant arrangement when a Business Associate Agreement is signed and system access stays under practice control, which matters more at check-out because it mixes health data with payment data.

Which wage sources sit behind this check-out coordinator comparison?

Wages come from the Bureau of Labor Statistics "Occupational Employment and Wage Statistics" program for May 2025, occupation code 43-6013, medical secretaries and administrative assistants, at a $45,930 median, with receptionists (code 43-4171, $38,010 median) noted as the lower proxy. Employer load percentages come from the same agency's "Employer Costs for Employee Compensation" series for March 2026, office and administrative support occupations in private industry, applied as separate components so paid leave and legally required benefits aren't counted twice. Cost per hire comes from SHRM's "2025 Benchmarking Report". Honest Taskers rates come from the company's own published rate card. Every wage here is a national median, so all of them move with your local wage band.

For this same cost stack run on a general administrative role rather than a healthcare one, see our virtual assistant vs in-house employee cost comparison.

Honest Taskers, HelpSquad Health, and My Mountain Mover lead this year's shortlist of virtual patient check-out coordinator companies, seven firms ranked on how they close a visit properly, from booking the follow-up before the patient leaves to collecting the balance, placing referrals and imaging orders, sending the visit summary, and reconciling what was charged against what was done. Check-out is the cheapest revenue in a practice and the most commonly skipped. Below we set out why a busy desk skips the half that can wait, whether card data is HIPAA or PCI, what patient-facing and back-office hours cost, whether one provider can do both halves, and how much last month left on the table.

Disclosure: Honest Taskers publishes this list and appears at position 1. We're judged on the same five published criteria as every other company here.

How we chose the best virtual patient check-out coordinator companies

We ranked these firms on five facts, read at each provider's own website on August 24, 2026: check-out scope, the HIPAA and Business Associate Agreement position in the firm's own words, published pricing, commitment terms, and the verified detail each publishes. One extra consideration matters here. Check-out often involves taking payment, which is card data rather than health data, and PCI obligations are separate from HIPAA. Only one firm in this wider pool names PCI-DSS at all, so if the role handles cards, that question needs asking directly.

Best virtual patient check-out coordinator companies compared

The table ranks all 7 on the same columns, Honest Taskers first because we publish the list. Facts were read on August 24, 2026 and change often, so treat them as a starting point, not a quote.

Virtual patient check-out coordinator companies compared on the criteria stated below, read at each provider's own site.

Notice that the useful firms for check-out are the ones with a billing capability as well as a front-desk one, because closing a visit is half scheduling and half money. HelpSquad Health prices both halves separately, which is unusually helpful. Staffingly is the cheapest published capacity and comes from the billing side rather than the reception side, which suits a practice whose check-out problem is uncollected balances rather than unbooked follow-ups.

1. Honest Taskers

Best for: practices where follow-ups go unbooked and balances become statements.

Pricing: $10.00–$12.65/hrCompliance: HIPAA verified by Accountable; BAACommitment: HourlyFirst hire: Two-week working trial

Check-out is where a practice quietly loses money it has already earned. The follow-up is not booked so the patient drifts. The balance is not collected at the desk so it becomes a statement, then a write-off. The referral is discussed and never placed. The charge does not match what was documented. Honest Taskers places staff working your time zone inside your practice management system on all of it: booking the follow-up while the visit is still live, calling patients about outstanding balances and taking them through payment, placing referral and imaging orders and confirming they were received, sending the visit summary and after-care instructions, and reconciling charges against the documented encounter so nothing is missed. Staff are trained on HIPAA and data privacy under a dedicated compliance officer, with a Business Associate Agreement signed before any patient data is touched and HIPAA compliance verified by Accountable. Rates run about $10 to $12.65 an hour by role, background, schedule and location, with a two-week working trial on the first hire. See the multi-purpose virtual medical assistant service for role detail.

Limitation: staff work remotely, so a payment taken at the window still needs somebody at the window, and the firm names no PCI-DSS or SOC 2 position of the kind MyOutDesk publishes.

2. HelpSquad Health

Best for: practices wanting one provider priced separately for patient calls and billing work.

Pricing: $8–10/hr digital; $10–13/hr voiceCompliance: BAA and virtual desktop includedCommitment: 20 hrs/wk, 3-month termSetup: $0 upfront

HelpSquad Health publishes the only full rate card in this pool. It is the only firm here that publishes a full rate card, splitting back-office digital work at $8 to $10 an hour from patient-facing voice work at $10 to $13, with managed teams quoted separately and shared live chat from $185 to $600 a month. What matters more is that the rate includes a HIPAA BAA and a virtual desktop, and it states individual HIPAA certification for every assistant. It is a US-based, US-managed company, charges $0 upfront, $0 implementation and $0 replacement costs, and names client practices including Imagine Advanced Dental Arts, Keady Family Practice and Spark Orthodontics.

Limitation: the 20 hours a week per assistant minimum and a 3-month agreement that auto-renews in 3-month increments make it the least flexible published commitment here, so it suits a settled need rather than a trial.

3. My Mountain Mover

Best for: practices that want a signed BAA in the standard contract and billing capability alongside reception.

Pricing: Not publicly listedCompliance: Signed BAA in every healthcare contractCommitment: Not publicly listedSpeed: Hired in 7–14 days

My Mountain Mover settles the compliance question in the contract rather than in negotiation. It gives the clearest contractual answer in this pool, stating that a signed HIPAA Business Associate Agreement is part of the contract provided to all healthcare clients rather than something you negotiate. Its talent is described as pre-trained healthcare staff experienced in widely used electronic health records, and it reports 1000 or more practices served, 1600 or more pre-trained virtual assistants and coverage of 50 or more specialties, with hiring completed within 7 to 14 days. It claims practices save up to 70% on overhead costs, a company-reported figure with no methodology published.

Limitation: it publishes no pricing at all, and the "save up to 70% on overhead costs" claim carries no published methodology, so treat it as marketing rather than a projection.

4. MEDVA

Best for: practices wanting a large, specialty-proven provider covering both reception and billing.

Pricing: Not publicly listedCompliance: HIPAA training validated by Compliancy GroupCommitment: None to beginScale: 1,000+ practices

MEDVA brings the largest verified practice base here. Its scale is the strongest verified claim in this pool: 1,000 or more healthcare practices, broken down as 400 or more medicine clinics, 200 or more surgical practices, 290 or more dental and orthodontic offices, and 75 or more physical therapy and chiropractic clinics. Staff include licensed medical assistants and certified medical scribes alongside receptionists, administrative assistants, billing assistants and remote patient monitoring support, with bilingual Spanish available. Screening is multi-stage with competency assessments, HIPAA and cybersecurity training is mandatory and validated through third-party compliance partner Compliancy Group, work happens remotely or in an optional Epic-approved secured facility, and a replacement VA is placed at no additional cost.

Limitation: no pricing is published anywhere, and its compliance position is a training claim validated by a third party rather than a stated Business Associate Agreement, so ask for the BAA explicitly.

5. Staffingly

Best for: practices whose check-out problem is uncollected balances, at the lowest published rate.

Pricing: $399/wk at 45 hrs; $299 at 10+Compliance: SOC 2 Type II, ISO 27001, signed BAACommitment: Flat weeklyDelivery: India, Pakistan, Bangladesh

Staffingly has the strongest documented security position in this pool. It has the strongest documented security position in this pool, reporting SOC 2 Type II, ISO/IEC 27001:2022, a signed Business Associate Agreement and a $5 million errors and omissions and cyber policy, all company-reported, and it publishes its rate: $399 a week per person for 45 hours, falling to $349 at five or more people and $299 at ten or more, which is under $9 an hour at the top tier. Its work spans billing, coding, prior authorization, front office and documentation, delivered from company-stated secured facilities in India, Pakistan and Bangladesh.

Limitation: its workflow is AI-assisted with a person verifying output rather than fully human, and offshore delivery means overlap with your working day has to be arranged as a shift rather than assumed.

6. DocVA

Best for: practices where check-out conversations run into medication and after-care questions.

Pricing: Not publicly listedCompliance: HIPAA training (company-reported)Commitment: Contract, no lock-inStaff: RNs, pharmacists, certified billers

DocVA hires from a different pool than most, which shows up on anything clinically adjacent. Its differentiator is who it hires: it states that every DocVA virtual medical assistant is an experienced healthcare professional, naming registered nurses, pharmacists, certified billers and experienced medical scribes among them. Commercially it is unusually clear, stating there is a contract but no lock-in period and that you can cancel at any time. Delivery is from the Philippines with the Data Privacy Act referenced, assistants are described as HIPAA-certified and handling protected health information to the same standard as in-office staff, and it reports over 1,000 virtual assistants placed across hundreds of practices with 150 or more practices served.

Limitation: no pricing is published, and while assistants are described as HIPAA-certified there is no Business Associate Agreement wording, which is the document that allocates liability.

7. Hello Rache

Best for: practices testing follow-up booking cheaply with no commitment.

Pricing: $9.50/hrCompliance: HIPAA-trained and certifiedCommitment: Cancel anytimeBase: US offices in Arizona

Hello Rache is the cheapest published route into this role. It publishes $9.50 USD per hour, the lowest openly stated rate among healthcare-specialized firms, on a "Cancel anytime - No Long-Term Commitment" basis, which together make it the cheapest way to test whether a remote hire works for your practice at all. Roles cover virtual medical scribe, virtual medical receptionist, virtual administrative assistant and virtual optometry assistant, with dental and veterinary specialisations alongside. It describes its assistants as HIPAA-trained and certified, and operates US offices in Arizona.

Limitation: it names no Business Associate Agreement anywhere on its pricing page, which for a firm handling protected health information is the gap to close in writing first, and its own placement and practice counters render as placeholder zeros.

Why does a busy desk skip the coordinator's half of check-out?

Because the half that can wait always does, and booking the follow-up, placing the referral and chasing the balance are the parts that wait. Split the role by what needs a physical presence. A card presented at the window needs somebody at the window. Booking the follow-up, placing the referral, chasing the balance afterwards, sending the summary and reconciling the charge do not, and they are the parts that get skipped when the desk is busy. Remote coverage is also cheaper: a medical secretary or administrative assistant earned a median $22.08 an hour, about $45,930 a year, according to the U.S. Bureau of Labor Statistics May 2025 wage data for secretaries and administrative assistants, against published rates from $9.50 an hour, or under $9 at Staffingly's top tier. The strongest argument for moving this work is that it is measurable. Follow-up booking rate, point-of-service collection rate and referral completion rate are all countable before and after, which makes this one of the few staffing decisions you can prove. For the scope, see tasks to outsource to a virtual medical assistant.

Is card data the coordinator handles HIPAA or PCI?

PCI, which is a separate regime from HIPAA and applies the moment payments are in scope. Check-out mixes health data with payment data, so both are live.

BAA versus HIPAA training. A Business Associate Agreement is a contract allocating liability for a breach. "HIPAA-trained" describes a person who sat a course. Ask which you are getting, in writing.
Card data is PCI, not HIPAA. If the role touches payments, ask about PCI-DSS separately. MyOutDesk is the only firm in this market naming it.
Collection tone. A balance call in your name can damage a relationship. Agree the script and the escalation point.
Referral follow-through. Placing an order is not the same as it being received. Ask how completion is confirmed.
Charge reconciliation authority. A coordinator can flag a mismatch; changing a charge is a billing decision. Write the boundary down.

What does a check-out coordinator cost, patient-facing or back office?

HelpSquad Health prices the two separately, $8 to $10 an hour for back office against $10 to $13 patient-facing, and check-out spans both. Four firms publish rates. Hello Rache is $9.50 an hour, cancel anytime. HelpSquad Health is $8 to $10 an hour for back-office work and $10 to $13 for patient-facing, with a BAA and virtual desktop included, but a 20 hours a week minimum on a rolling 3-month term. Staffingly is $399 a week per person for 45 hours, falling to $299 at ten or more, under $9 an hour at the top tier. Honest Taskers is $10.00 to $12.65 an hour with no minimum. Everyone else on this list quotes on request. For context, a medical secretary or administrative assistant earned a median $22.08 an hour, about $45,930 a year, according to the U.S. Bureau of Labor Statistics May 2025 wage data for secretaries and administrative assistants. For check-out the number to watch is point-of-service collection rate and follow-up booking rate, both of which you can measure before and after. A practice collecting even a few hundred dollars more a week covers this work several times over. Rates were checked on August 24, 2026 and change often. For rate context, see how much a virtual medical assistant costs in 2026.

Can the coordinator do both halves, scheduling and money?

Check-out is scheduling and money together, so ask whether the provider genuinely does both. Then a signed BAA, a PCI position if payments are involved, and measurable targets.

Both halves of the role. Check-out is scheduling and money. Ask whether the provider genuinely does both.
A signed BAA. Three firms here state one; the rest describe training.
PCI position for payments. Ask explicitly if card data is in scope.
Measurable targets. Agree follow-up booking rate and collection rate before starting.
Referral confirmation process. Ask how the provider verifies a referral was received.

How much did check-out coordination leave on the table last month?

Measure one month of follow-ups booked at the visit, balances collected at the point of service, and referrals placed and confirmed, because check-out failures are unusually easy to quantify.

Measure one month: follow-ups booked at visit, balances collected at point of service, referrals placed and confirmed.
Split the tasks into window work and remote work.
Decide whether payments are in scope, which decides whether PCI matters.
Get the BAA and scoped system access in place before the first shift.
Review at four weeks against the same three numbers.

Methodology and sources

We read each company's own website on August 24, 2026, recording check-out scope, the HIPAA and Business Associate Agreement position exactly as each firm words it, published pricing, commitment terms, and the facts each firm publishes about itself. Wage context is from the U.S. Bureau of Labor Statistics May 2025 Occupational Employment and Wage Statistics. Business Associate Agreement obligations are set by the HIPAA Privacy and Security Rules. Where a company didn't publish a fact we marked it "not publicly listed"; practice counts, retention rates and savings claims are company-reported and were not independently audited. Portiva was excluded because its site redirects to a domain that refuses automated requests.

If follow-ups are going unbooked, that is the cheapest revenue in the practice to recover. Our guide to virtual medical assistant duties breaks the role down task by task and shows which parts of check-out can move off the desk entirely.
