Q: Can a virtual therapy practice assistant handle insurance verification and superbills?

Yes, both are administrative tasks that sit in the remote column. The assistant checks behavioral health benefits before a first session, confirms copays and deductibles, and flags in-network versus out-of-network status. For out-of-network clients it prepares superbills to submit for reimbursement and can file claims and track the explanation of benefits. Coding judgment stays with the practice unless a coder is scoped into the role.

Q: How fast can a virtual therapy practice assistant start compared with an in-house hire?

Most Honest Taskers placements complete within one to three weeks of a signed agreement, and the first hire comes with a two-week working trial. Recruiting an in-house administrative person in most US markets takes longer than that before onboarding even begins. Honest Taskers reports 99.6% average monthly retention, and replacements run through the same provider rather than a fresh recruiting cycle. An in-house departure restarts recruiting and onboarding from zero.

Is a virtual therapy practice assistant cheaper than in-house front desk staff?

Yes, on the administrative hours that move, because a remote assistant bills $10.00 to $12.65 an hour with none of the employer load a salaried hire carries. An in-house administrative person at the $45,930 median costs about $68,252 all-in once insurance, paid leave, and required benefits are added. The saving applies only to the software-based work you shift, not to your whole payroll, so run the math on your own local wages. Anything that needs a person in the office still stays in-house.

What can't a virtual therapy practice assistant do at a therapy practice?

A virtual therapy practice assistant can't do the work that happens in person. That means greeting clients in the waiting room, collecting a copay or sliding-scale payment in cash, handling paper intake and release of information forms, and prepping the office between sessions. Any clinical decision also stays with your licensed therapists. Everything that lives in your scheduling and billing software can move to the remote seat.

Should a solo therapy practice use a virtual therapy practice assistant instead of hiring in-house?

Yes, a solo practice usually gets more from a part-time remote assistant than a full-time in-house hire. One caseload rarely justifies a salaried front desk, yet admin work still eats into billable hours or spills into the evening. A remote seat covers scheduling, reminders, and verification at an hourly rate with no floor beneath it. Keep any in-person task, such as collecting payment at the office, with yourself or an on-site helper.

Do I have to choose between in-house staff and a virtual therapy practice assistant?

No, most growing practices run both. The workable pattern keeps in-house staff for the waiting room, in-person payments, and paper intake, then moves phone, verification, billing, and portal work to a remote seat. That's augmentation rather than replacement, and it gives your on-site person their day back. Move a queue rather than a whole role, so the face-to-face half always has someone covering it.

What makes therapy practice admin different from general medical admin?

Authorized visit counts. Therapy care is delivered as a plan of a set number of visits, each of which must be authorized, tracked and reauthorized before the count runs out. Deliver a visit outside an active authorization and it is unpaid; run out mid-plan and the course of care is interrupted.

Which providers are built for therapy practices?

Rockstar Global names physical therapy, occupational therapy, speech-language pathology and dental service organizations as the practice types it serves, and staffs verification and authorization specialists as a separate role. MEDVA reports 75 or more physical therapy and chiropractic clinics among its 1,000-plus practices.

When should reauthorization be started?

Before the visit count runs out, with two or three visits still remaining rather than at the last one. Agree the trigger point in writing at the start of the engagement, because this single habit is what separates a therapy practice that completes plans of care from one that keeps interrupting them.

Can a virtual assistant write progress notes?

No, chasing outstanding notes so claims can go out is administrative work a remote assistant can own. Writing the clinical content of a progress note is the therapist's responsibility, and it gates the claim, so the useful division is that the assistant tracks and chases while the therapist authors.

How much does a virtual medical assistant for therapy practice cost?

Honest Taskers publishes $10.00 to $12.65 an hour and HelpSquad Health $8 to $13 depending on whether the work is back-office or patient-facing. Rockstar Global, MEDVA, My Mountain Mover, DocVA and Virtual Nurse Rx all quote on request.

How should same-day cancellations be handled?

With a real waitlist process rather than an intention. An unfilled same-day slot in a therapy practice is lost revenue that cannot be recovered, so ask any provider specifically how a cancellation gets backfilled, how quickly, and from what list.

Do therapy practice assistants need a Business Associate Agreement?

Yes, they handle authorizations, diagnoses and progress note tracking. Honest Taskers, My Mountain Mover and Virtual Nurse Rx state a signed BAA, and HelpSquad Health includes one in its rate. Rockstar Global, MEDVA and DocVA describe HIPAA compliance or training instead.

Can one assistant cover authorization and scheduling?

For a small practice, usually yes, and it is often better because the person tracking visit counts is also the person booking them. Above a certain volume the authorization work justifies its own role, which is how Rockstar Global structures it.

Choosing between a virtual therapy practice assistant and in-house staff is a work-allocation decision before it's a budget one. Where a virtual therapy practice assistant fits against in-house staff comes down to presence, so the honest starting point is what only in-house staff can do, which is every task that happens face to face at a therapy practice. Once that's fixed, it's worth mapping the scheduling and intake work that shifts to a remote seat, how that seat cuts no-shows and late cancellations, and whether it can handle insurance verification and superbills. Cost follows scope rather than the reverse. What in-house staff cost a therapy practice each year runs well past the salary line once the employer load is added, while a virtual therapy practice assistant costs an hourly rate with no load at all. From there the questions get practical: why solo and group practices weigh the choice differently, how soon a remote assistant can start, and what breaks when in-house staff cover the front desk alone. The last stretch is about the call itself, how a therapy practice should choose one over the other, when it keeps in-house staff and adds a remote seat, and where these cost figures come from.

Where does a virtual therapy practice assistant fit against in-house staff?

A virtual therapy practice assistant fits wherever the work lives in software rather than in the waiting room. The role is a healthcare-trained remote professional working inside your existing systems on administrative and clinically adjacent tasks. In-house staff are the people in the office who greet clients, cover the front desk, and do anything the building requires. The real difference isn't skill, it's presence. Anything that needs a body on site stays in-house, and anything that needs only access to your scheduling and billing software can move to a remote seat.

That split also decides the money. An employee costs a salary plus the load stacked on top, whether or not there's a full week of work to fill it. A remote assistant costs an hourly rate for the hours you use. Comparing a $22 in-house hour against a $12 remote hour misreads both sides, because the true employer cost sits well above $22 and the remote cost has no floor under it.

What can only in-house staff do at a therapy practice?

Only in-house staff can do the work that needs a person physically in the office, and that limit belongs before any cost table. A virtual therapy practice assistant can't cover the parts of a therapy practice that happen face to face. In-house staff keep the following on-site tasks.

Greet clients in the waiting room, check them in, and settle anyone who arrives distressed.
Collect a copay or a sliding-scale payment in person and hand over a receipt.
Handle paper intake packets, release of information (ROI) forms, and physical mail that lands at the office.
Prep the office and therapy rooms, manage keys and supplies, and keep the space private between sessions.
Make any clinical call, which stays with your licensed therapists wherever they sit.

Where most of an open role sits on that list, the comparison is already settled and you're hiring in-house. Read on where a real share of the work is administrative, which in most therapy practices it is, because front-desk staff absorb scheduling, billing, and portal work simply by being the ones present when it arrives.

What scheduling and intake work shifts to a virtual therapy practice assistant?

The scheduling and intake work that shifts is the work living in your practice management software, so deciding what to outsource starts with a single test. A virtual therapy practice assistant can take on booking and rescheduling, filling cancellations from the waitlist, new-client intake and portal onboarding, insurance verification, appointment reminders, and recall outreach to clients who lapsed. For telehealth and hybrid practices it also covers sending the session link, confirming the platform works, and chasing intake forms before the first appointment. Software such as SimplePractice, TherapyNotes, and IntakeQ is where most of this happens, and a healthcare-trained assistant can be matched to the system you already run. For the task-by-task breakdown, see our list of tasks to outsource to a virtual medical assistant. One line to hold: documentation support means preparing and drafting notes, never deciding what belongs in a clinical record, and that boundary goes in the role description rather than getting settled during onboarding.

How does a virtual therapy practice assistant cut no-shows and late cancellations?

A virtual therapy practice assistant cuts no-shows and late cancellations by running the reminder and confirmation cycle that a busy front desk drops first. Reminders go out on a schedule across text, email, and phone, confirmations get logged, and anyone who cancels late gets offered the next open slot while the waitlist is worked to backfill the hour. The assistant can also apply your no-show and late-cancel fee policy the same way every time, flagging the charge and the note for the therapist rather than deciding anything about the client relationship. That consistency is the point. A single front-desk person handles reminders when the lobby is quiet and skips them when it isn't, so the gaps appear exactly when the schedule is fullest. Moving the cycle to a remote seat means it runs the same on a heavy day as a light one, and an empty therapy hour that can't be billed is the most expensive thing on the calendar.

Can a virtual therapy practice assistant handle insurance verification and superbills?

Yes, a virtual therapy practice assistant can handle insurance verification and superbills, both of which are administrative tasks that sit squarely in the remote column. Before a first session the assistant checks behavioral health benefits, confirms the copay and deductible, and flags whether the client is in or out of network. For out-of-network clients it prepares superbills the client can submit for reimbursement, and it can generate and send claims, track the explanation of benefits, and follow up on anything that stalls. Sliding-scale arrangements get documented the same way, so the agreed rate is recorded and applied without the therapist chasing paperwork. There's a line here worth writing down. Verification and superbill preparation are administrative, so coding judgment and anything that changes a client's clinical record stay with the practice unless a coder is scoped into the role deliberately. Set that in the role description so it doesn't get decided hour by hour.

What does in-house staff cost a therapy practice each year?

In-house staff cost a therapy practice far more than the salary line shows once the employer load is added. US medical secretaries and administrative assistants earned a median $45,930 a year (Source: Bureau of Labor Statistics, "Occupational Employment and Wage Statistics", May 2025, code 43-6013). The load on top is broken out separately in the table below so nothing is counted twice (Source: Bureau of Labor Statistics, "Employer Costs for Employee Compensation", March 2026).

What one in-house administrative hire costs a US therapy practice per year at the national median wage.
Cost line	What it covers	On top of wages	Per year
Base salary	The advertised pay for the role	n/a	$45,930
Insurance	Health and related coverage	17.5%	$8,038
Paid leave	Vacation, sick days and holidays	11.9%	$5,466
Legally required	Employer FICA, unemployment, workers' compensation	10.2%	$4,685
Supplemental pay	Overtime, bonuses and shift differentials	4.5%	$2,067
Retirement and savings	Employer contributions and match	4.5%	$2,067
All-in recurring	What the seat costs before equipment or space	48.7%	about $68,252

That table is recurring cost only, and a few things sit outside it. Filling the seat adds an average $5,475 per hire for non-executive roles (Source: SHRM, "2025 Benchmarking Report"), and that cost returns on every turnover. Equipment, software seats, and office space sit on top and vary too much between practices to carry one national figure. Coverage is the quietest gap of all, because a single front-desk hire is one person, so when they take leave the work stops or lands on your therapists, and that operational hole appears nowhere in the numbers above.

What does a virtual therapy practice assistant cost?

A virtual therapy practice assistant costs $10.00 to $12.65 an hour, billed hourly with no weekly minimum. None of the employer load applies. At 40 hours a week that works out to about $20,800 to $26,312 a year, and 20 hours a week runs about $10,400 to $13,156. You're buying hours rather than employing a person, so there are no payroll taxes, no benefits, no paid leave, and no workspace to fund. The part-time figure is the one most therapy practices underweight. An in-house administrative hire is usually a full-time decision even when the real work is 20 hours, because half-time front-desk roles are hard to fill and harder to keep. Hourly billing removes that floor. Work out your own number rather than trusting either figure: total your real loaded in-house cost using local wages, then price the same hours at $10.00 to $12.65, remembering the difference applies only to the administrative hours that move. For the pricing detail, see our guide to how much a virtual medical assistant costs.

Why do solo and group practices weigh a virtual therapy practice assistant differently?

Because the two carry different overhead, solo and group therapy practices land in different places on the same decision. A solo clinician often has no front desk at all, so admin work eats directly into billable hours or spills into the evening. Hiring a full-time in-house person rarely pencils out for one caseload, which is exactly where a part-time remote seat fits, covering scheduling, reminders, and verification without the cost of a salaried person. Group practices already run a front desk, so the question shifts from whether to hire to how the queues get split. The pattern that works keeps a lean in-house coordinator for the face-to-face layer and moves phone, portal, and billing work to one or two remote assistants, which often covers what used to take three seats. When you're comparing providers for that remote layer, see our roundup of the best virtual therapy practice assistant companies before you shortlist.

How soon can a virtual therapy practice assistant start?

A virtual therapy practice assistant reaches the desk sooner than an in-house hire in most cases. Most Honest Taskers placements complete within one to three weeks of a signed agreement, and the first hire comes with a two-week working trial, so you test the fit on real work before committing further. Recruiting an in-house administrative person in most US markets takes longer than that before onboarding even begins, and the seat sits empty while the work piles onto whoever is already there. Turnover is the other half of the timing. Honest Taskers reports 99.6% average monthly retention, and where a placement doesn't fit, the replacement runs through the same provider rather than a fresh recruiting cycle you have to staff yourself. An in-house departure restarts recruiting, onboarding, and the ramp from zero, so price that cycle honestly when you weigh the two options, because it lands whether or not you budgeted for it.

What breaks when in-house staff cover the therapy front desk alone?

When in-house staff cover the therapy front desk alone, the work breaks at the busiest moments, which are the moments it matters most. One person answering the phone, checking clients in, and verifying benefits does whichever task is loudest, so reminders get skipped, the waitlist goes unworked, and superbills stack up until someone finds an hour. No-shows climb because the confirmation cycle only runs when the lobby is quiet. New-client intake and portal onboarding slip, which pushes back first sessions and the revenue attached to them. Then there's coverage: one front-desk person is a single point of failure, so a sick day or a resignation stops the queue cold or dumps it on your therapists. None of this means the in-house seat is the problem. It means one seat can't hold both the face-to-face layer and every software queue at once, and the software queue is the half that quietly loses.

How should a therapy practice choose a virtual therapy practice assistant over in-house staff?

A therapy practice should choose between the two by sorting the open role into two columns before pricing anything, because that split decides more than any rate card. The first column holds every task that needs someone physically in the office. Everything that needs only access to your systems goes in the second. Then run four tests against the columns, in order, because each one can settle the decision on its own.

How big is the on-site column? Where it holds most of the role, hire in-house and stop.
Does the remote column fill a full week? Where it doesn't, an hourly seat fits a workload no salaried hire can be sized to.
How urgent is the gap? Weeks against months changes the answer by itself.
What breaks when whoever covers a column is out? Coverage is a cost even when it isn't on the invoice.

Where you're unsure the workload justifies either route, our guide to the signs your practice needs a virtual assistant helps size it before you commit.

When does a practice keep in-house staff and add a virtual therapy practice assistant?

A practice keeps in-house staff and adds a virtual therapy practice assistant when the office still needs a person on site but the software queues have outgrown that one seat. That's most growing practices, because the question was never either-or. The pattern that holds keeps in-house staff for the waiting room, in-person payments, paper intake, and the office itself, then moves phone coverage, verification, billing, and portal onboarding to a remote seat. It's augmentation rather than replacement, and it shows up first as your on-site person getting their day back instead of drowning in callbacks. Watch for the tell: an in-house employee spending hours a day on work that never needed the building. When that's happening you're paying a loaded salary for output an hourly remote seat could deliver, while the person you hired for the front desk isn't free to do the part only they can do.

Where do these virtual therapy practice assistant cost figures come from?

These cost figures come from named public sources rather than estimates. Wages come from the Bureau of Labor Statistics "Occupational Employment and Wage Statistics" program for May 2025, occupation code 43-6013, medical secretaries and administrative assistants. The employer load percentages come from the same agency's "Employer Costs for Employee Compensation" series for March 2026, office and administrative support occupations, applied as separate components so paid leave and legally required benefits aren't double counted. Cost per hire comes from SHRM's "2025 Benchmarking Report". Honest Taskers rates come from the company's own published rate card rather than a third-party estimate. Every wage and load figure here is a national median, so all of them move with your local market. Run the math on your own therapy practice rather than take a national number on trust.

For the same cost comparison run on a general administrative role rather than a therapy one, see our virtual assistant vs in-house employee cost comparison.

Honest Taskers, Rockstar Global, and MEDVA lead this year's shortlist of virtual therapy practice assistant companies, seven firms ranked on how they support physical, occupational, speech and behavioral therapy practices, from authorization and visit-limit tracking to scheduling recurring plans of care, cancellation backfill, progress note chasing, and the reauthorization that has to happen before the visits run out. Therapy practices run on authorized visit counts, which makes this a different administrative job from general medicine.

Disclosure: Honest Taskers publishes this list and appears at position 1. We're judged on the same five published criteria as every other company here.

How we chose the best virtual therapy practice assistant companies

We ranked these firms on five facts, read at each provider's own website on August 24, 2026: therapy practice scope, the HIPAA and Business Associate Agreement position in each firm's own words, published pricing, commitment terms, and the verified detail each publishes. The criterion that matters most is whether the provider understands visit authorization, because a therapy practice does not fail on booking, it fails on running out of authorized visits mid-plan or delivering care against an expired authorization. One firm here is built specifically for therapy practices and names verification and authorization as a separate role.

Best virtual therapy practice assistant companies compared

The table ranks all 7 on the same columns, Honest Taskers first because we publish the list. Facts were read on August 24, 2026 and change often, so treat them as a starting point, not a quote.

Virtual therapy practice assistant companies compared on the criteria stated below, read at each provider's own site.

One firm on this list was built for this exact market. Rockstar Global names physical therapy, occupational therapy, speech-language pathology and dental service organizations as the practice types it serves, and it staffs verification and authorization specialists as a separate role rather than as a task an assistant picks up. MEDVA is the useful second reference point, reporting 75 or more physical therapy and chiropractic clinics among its 1,000-plus practices.

1. Honest Taskers

Best for: therapy practices losing visits to expired authorizations and unfilled cancellations.

Pricing: $10.00–$12.65/hrCompliance: HIPAA verified by Accountable; BAACommitment: HourlyFirst hire: Two-week working trial

A therapy practice has an administrative failure mode general medicine does not: the authorized visit count. Care is delivered in a plan of eight or twelve or twenty visits, and every one of those has to be authorized, tracked and reauthorized before it runs out. Get it wrong and you have either delivered unpaid care or interrupted a plan halfway through, and both cost more than the admin would have. Honest Taskers places staff working your time zone inside your systems on that whole cycle: verifying benefits and authorized visit counts before the plan starts, tracking visits used against visits authorized, starting reauthorization before the count is exhausted rather than after, scheduling recurring appointments as a block so the plan completes, backfilling cancellations from a waitlist the same day, and chasing outstanding progress notes so claims can go out. Staff are trained on HIPAA and data privacy under a dedicated compliance officer, with a Business Associate Agreement signed before access and HIPAA compliance verified by Accountable. Rates run about $10 to $12.65 an hour by role, background, schedule and location, with a two-week working trial on the first hire. See the virtual mental health assistant service for behavioral therapy detail.

Limitation: staff work administratively, so clinical decisions about the plan of care and the content of progress notes stay with your therapists, and the firm describes its security environment as SOC 2 audit ready rather than holding a SOC 2 or ISO 27001 certification.

2. Rockstar Global

Best for: PT, OT and SLP practices wanting a provider built for their market with named authorization specialists.

Pricing: Not publicly listedCompliance: States 100% HIPAA-compliantCommitment: 7–14 day implementationFocus: PT, OT, SLP and DSOs

Rockstar Global is the only firm here built around specific practice types rather than healthcare generally. Formerly Virtual Rockstar, it is the only firm in this pool built around a specific set of practice types rather than healthcare generally, naming physical therapy, occupational therapy, speech-language pathology and dental service organizations. Its roles reflect that focus: medical reception specialists, verification and authorization specialists, medical billing specialists, administrative support specialists and credentialing specialists, supported by an internal VA Academy for ongoing development. It reports placing its 300th VA across more than 170 private practices with a 97% practice partner retention rate, around $20,000 saved in annual profits per solution as a company-reported figure, and a 7 to 14 day average implementation.

Limitation: it publishes no pricing and no Business Associate Agreement wording, and its focus on therapy practices and dental service organizations means a general medical practice is not its core buyer.

3. MEDVA

Best for: therapy clinics wanting a provider with a proven base in PT and chiropractic.

Pricing: Not publicly listedCompliance: HIPAA training validated by Compliancy GroupCommitment: None to beginScale: 1,000+ practices

MEDVA brings the largest verified practice base here. Its scale is the strongest verified claim in this pool: 1,000 or more healthcare practices, broken down as 400 or more medicine clinics, 200 or more surgical practices, 290 or more dental and orthodontic offices, and 75 or more physical therapy and chiropractic clinics. Staff include licensed medical assistants and certified medical scribes alongside receptionists, administrative assistants, billing assistants and remote patient monitoring support, with bilingual Spanish available. Screening is multi-stage with competency assessments, HIPAA and cybersecurity training is mandatory and validated through third-party compliance partner Compliancy Group, work happens remotely or in an optional Epic-approved secured facility, and a replacement VA is placed at no additional cost.

Limitation: no pricing is published anywhere, and its compliance position is a training claim validated by a third party rather than a stated Business Associate Agreement, so ask for the BAA explicitly.

4. My Mountain Mover

Best for: therapy practices that want a signed BAA in the standard contract plus billing capability.

Pricing: Not publicly listedCompliance: Signed BAA in every healthcare contractCommitment: Not publicly listedSpeed: Hired in 7–14 days

My Mountain Mover settles the compliance question in the contract rather than in negotiation. It gives the clearest contractual answer in this pool, stating that a signed HIPAA Business Associate Agreement is part of the contract provided to all healthcare clients rather than something you negotiate. Its talent is described as pre-trained healthcare staff experienced in widely used electronic health records, and it reports 1000 or more practices served, 1600 or more pre-trained virtual assistants and coverage of 50 or more specialties, with hiring completed within 7 to 14 days. It claims practices save up to 70% on overhead costs, a company-reported figure with no methodology published.

Limitation: it publishes no pricing at all, and the "save up to 70% on overhead costs" claim carries no published methodology, so treat it as marketing rather than a projection.

5. DocVA

Best for: therapy practices whose authorization work needs clinical context assembled properly.

Pricing: Not publicly listedCompliance: HIPAA training (company-reported)Commitment: Contract, no lock-inStaff: RNs, pharmacists, certified billers

DocVA hires from a different pool than most, which shows up on anything clinically adjacent. Its differentiator is who it hires: it states that every DocVA virtual medical assistant is an experienced healthcare professional, naming registered nurses, pharmacists, certified billers and experienced medical scribes among them. Commercially it is unusually clear, stating there is a contract but no lock-in period and that you can cancel at any time. Delivery is from the Philippines with the Data Privacy Act referenced, assistants are described as HIPAA-certified and handling protected health information to the same standard as in-office staff, and it reports over 1,000 virtual assistants placed across hundreds of practices with 150 or more practices served.

Limitation: no pricing is published, and while assistants are described as HIPAA-certified there is no Business Associate Agreement wording, which is the document that allocates liability.

6. Virtual Nurse Rx

Best for: therapy practices needing authorization cover within days on a month-to-month basis.

Pricing: Not publicly listedCompliance: BAA provided to every clientCommitment: Month-to-monthSpeed: 48-hour average deployment

Virtual Nurse Rx is the fastest to deploy and the most explicit about scope. Two published statements make it worth close attention. The first is commercial: it provides a BAA to every client, on month-to-month engagements, with an average deployment of 48 hours for a ready practice. The second is a piece of honesty rare in this market. Despite RN and MD staffing in its own positioning, it states plainly that it offers administrative support only and that clinical decisions always remain with your licensed providers. It also advertises a "100% Human-Performed Service: No AI Does Your Work", which is a direct answer to a question more practices are asking. It is headquartered in Sun City Center, Florida and serves all 50 states.

Limitation: no pricing is published, and the RN and MD framing in its own positioning sits awkwardly beside its statement that the service is administrative support only, so be clear internally about what you are buying.

7. HelpSquad Health

Best for: therapy practices wanting published rates across both front desk and billing.

Pricing: $8–10/hr digital; $10–13/hr voiceCompliance: BAA and virtual desktop includedCommitment: 20 hrs/wk, 3-month termSetup: $0 upfront

HelpSquad Health publishes the only full rate card in this pool. It is the only firm here that publishes a full rate card, splitting back-office digital work at $8 to $10 an hour from patient-facing voice work at $10 to $13, with managed teams quoted separately and shared live chat from $185 to $600 a month. What matters more is that the rate includes a HIPAA BAA and a virtual desktop, and it states individual HIPAA certification for every assistant. It is a US-based, US-managed company, charges $0 upfront, $0 implementation and $0 replacement costs, and names client practices including Imagine Advanced Dental Arts, Keady Family Practice and Spark Orthodontics.

Limitation: the 20 hours a week per assistant minimum and a 3-month agreement that auto-renews in 3-month increments make it the least flexible published commitment here, so it suits a settled need rather than a trial.

Should therapy practices staff authorization work in-house or with a remote assistant?

Remotely, because authorization work is portal and phone work with hard deadlines, and it is the task most often squeezed out by a busy clinic day. The cost case is clear: a medical secretary or administrative assistant earned a median $22.08 an hour, about $45,930 a year, according to the U.S. Bureau of Labor Statistics May 2025 wage data for secretaries and administrative assistants, against published rates from $9.50 an hour. The stronger argument is specialisation. A therapy practice needs somebody who understands that visits are a currency, watches the count, and starts reauthorization at visit six of eight rather than visit eight. That is a habit, and it is easier to buy from a provider whose staff already have it. Rockstar Global is the only firm here built around this market, and MEDVA has the largest verified therapy client base. What stays with your therapists is the plan of care itself and every word of the progress note. For the scope, see tasks to outsource to a virtual medical assistant.

What are the risks of outsourcing therapy practice admin to an assistant?

In therapy the expensive mistakes are all timing mistakes, and they surface weeks later as denials.

BAA versus HIPAA training. A Business Associate Agreement is a contract allocating liability for a breach. "HIPAA-trained" describes a person who sat a course. Ask which you are getting, in writing.
Visits delivered against an expired authorization. Ask for the tracking report showing visits used against visits authorized, and check it weekly.
Reauthorization started too late. Agree the trigger point, ideally with two or three visits still remaining.
Progress notes gating claims. Chasing notes is administrative; writing them is not. Write the boundary down.
Cancellation backfill speed. A same-day unfilled slot is lost revenue. Ask what the waitlist process is.

How much does a therapy practice assistant cost?

Four firms publish rates. Hello Rache is $9.50 an hour, cancel anytime. HelpSquad Health is $8 to $10 an hour for back-office work and $10 to $13 for patient-facing, with a BAA and virtual desktop included, but a 20 hours a week minimum on a rolling 3-month term. Staffingly is $399 a week per person for 45 hours, falling to $299 at ten or more, under $9 an hour at the top tier. Honest Taskers is $10.00 to $12.65 an hour with no minimum. Everyone else on this list quotes on request. For context, a medical secretary or administrative assistant earned a median $22.08 an hour, about $45,930 a year, according to the U.S. Bureau of Labor Statistics May 2025 wage data for secretaries and administrative assistants. For a therapy practice the number to watch is authorized visits delivered as a share of visits authorized, plus cancellation backfill rate. Recovering a handful of visits a week covers this cost comfortably. Rates were checked on August 24, 2026 and change often. For rate context, see how much a virtual medical assistant costs in 2026.

What should you look for in a therapy practice assistant?

Authorization discipline, therapy market experience, a signed BAA, and a real backfill process.

Authorization named as a capability. Rockstar Global staffs it as a role. Ask the others who does it and how it is tracked.
Therapy market experience. Ask how many PT, OT or SLP practices the provider works with.
A visit-count tracking report. Ask to see it before signing, not after.
A signed BAA. Three firms here state one.
Backfill process for cancellations. Ask specifically how a same-day cancellation gets filled.

How do you set up a therapy practice assistant?

Start with the authorization report, because that is where therapy practices lose money without noticing.

Audit one quarter: visits authorized, visits delivered, and any delivered outside an active authorization.
Count same-day cancellations and how many were backfilled.
Set the reauthorization trigger point in writing before shortlisting.
Get the BAA and payer portal access ready before the first shift.
Review at a quarter on visits delivered against authorized and backfill rate.

Methodology and sources

We read each company's own website on August 24, 2026, recording therapy practice scope, the HIPAA and Business Associate Agreement position exactly as each firm words it, published pricing, commitment terms, and the facts each firm publishes about itself. Wage context is from the U.S. Bureau of Labor Statistics May 2025 Occupational Employment and Wage Statistics. Business Associate Agreement obligations are set by the HIPAA Privacy and Security Rules. Where a company didn't publish a fact we marked it "not publicly listed"; practice counts, retention rates and savings claims are company-reported and were not independently audited. Portiva was excluded because its site redirects to a domain that refuses automated requests.

If plans of care are stalling before completion, the cause is usually authorization timing rather than patient commitment. Our guide to virtual medical assistant duties shows which parts of the authorization and scheduling cycle a remote hire can own outright.
